摘要
目的:比较针刺星状神经节与常规针刺治疗对后循环缺血的临床疗效差异及对血压的影响。方法:将80例后循环缺血患者随机分为观察组(40例)和对照组(40例)。观察组予以针刺颈部的双侧星状神经节,行捻转补法;对照组实证取风池、百会、内关、太冲,用泻法,虚证取风池、百会、肝俞、肾俞、足三里,用平补平泻法和补法,两组均3天治疗1次,共治疗4次。比较两组治疗前后证候总积分、椎动脉及基底动脉收缩期峰值血流速度(Vp)与收缩压、舒张压变化情况,并比较两组临床疗效。结果:治疗后两组证候总积分较治疗前均明显下降(均P<0.01),观察组较对照组下降更明显(P<0.01);观察组总有效率为87.5%(35/40),高于对照组的67.5%(27/40,P<0.05);治疗后,除了对照组椎动脉血流速度减慢型Vp较治疗前无明显改善(P>0.05),其余两组椎动脉、基底动脉血流速度异常者(包括血管痉挛或狭窄型和血流速度减慢型)的Vp均较治疗前改善(P<0.01),其中观察组均较对照组改善更明显(P<0.01);两组治疗后收缩压、舒张压均较治疗前下降(均P<0.01),其中观察组收缩压、舒张压下降均较对照组明显(均P<0.01)。结论:针刺星状神经节治疗后循环缺血疗效满意,降压效果显著,优于常规针刺。
ABSTRACT Objective To compare the difference in the clinical efficacy on posterior circulation ischemia be- tween acupuncture at stellate ganglion and conventional acupuncture as well as the impacts on blood pressure. Methods Eighty cases of posterior circulation ischemia were randomized into an observation group （40 cases） and a control group （40 cases）. In the observation group, acupuncture was applied to the bilateral stellate ganglions on the neck, stimulated with reinforcing technique by rotating the needles. In the control group, the acupuncture of reducing technique was applied to Fengchi （GB 20）, Baihui （GV 20）, Neiguan （PC 6） and Taichong （LR 3） in the excess syndrome. The even needling or reinforcing technique was applied to Fengehi （GB 20）, Baihui （GV 20）, Ganshu （BL 18）, Shenshu （BL 23） and Zusanli （ST 36） for the deficiency syndrome. The treatment was given once every 3 days and 4 treatments were required totally in the two groups. The changes in total syndrome score, peak systolic blood flow velocity （Vp） of vertebral artery and basilar artery, systolic and diastolic blood pressures were compared before and after treatment in the two groups. The clinical efficacy was compared between the two groups. Results The total syndrome score was reduced apparently after treatment compared with that before treatment in the two groups （P〈O. 01）, and the reducing was more obvious in the observation group as compared with that in the control group （P〈0.01）. The total effective rate was 87.5% （35/40） in the observation group, higher than 67.50% （27/40, P〈0.05） in the control group. After treatment, the reduced Vp of vertebral artery was not improved apparently as compared with that before treatment in the control group, Vp in blood velocity ab- normality （including vascular spasm, stenosis or reduced velocity） of vertebral artery and basilar artery was all im- proved as compared with that before treatment in the two groups （P〈0.01）, an
