Clinical studies of Q-switched Nd:YAG laser combined with recombinant human acidic fibroblast in treatment with facial melasma

摘要 目的:评价Q开关钕∶钇铝石榴石激光联合重组人酸性成纤维细胞生长因子(recombinant human acidic fibroblast growth factor,rh-aFGF)治疗面部黄褐斑的有效性与安全性.方法:2 012年2月-2014年3月,入选78例面部重度黄褐斑患者,对照组采用波长为1 064nm的掺钕钇铝石榴石(Neodymium-doped Yttrium Aluminium Garnet,Nd∶YAG)激光治疗;治疗组在对照组基础上给予rh-aFGF治疗,1次/月,连续治疗3次后观察黄褐斑的临床治疗效果及激光术后并发症的发生.结果:采用rh-aFGF干预后,治疗组与对照组在术后水肿(1.6±0.5 vs 2.2±0.5)、渗出(1.3±0.3 vs 1.8±0.4)、红斑(1.6±0.3vs 2.5±0.7)、疼痛灼烧评分(1.7±0.6 vs 2.7±0.8)、感染发生率(2.6% vs 11.5%)、皮肤弹性增加(25.8±4.7% vs 5.2±2.2%)、皮肤含水增加(55.8±14.6% vs 27.6±6.8%)、色素评分(1.9±0.5 vs 2.7±0.5)、完全愈合时间(4.2±1.6 vs 7.7±2.4)方面,两组比较具有统计学差异(P<0.05).结论:rh-aFGF联合Q开关Nd∶YAG激光治疗黄褐斑,可以减少激光术后并发症,增加皮肤弹性及含水量,减少色素沉着的发生. Objective To evaluate the Q-switched Nd∶YAG laser combined with recombinant human acidic fibroblast growth factor (rh-aFGF) treatment of facial melasma efficacy and safety.Methods February 2012-March 2014 selected 78 cases of severe facial melasma patients.The control group received a wavelength of 1064nmNd∶ YAG laser treatment.The treatment group received rh-aFGF treatment on the basis of the control group.Laser surgery groups were given antibiotics,ice therapy.1 times/month,the occurrence of melasma clinical outcomes and postoperative complications after laser treatment for 3 consecutive observations.Results After using rh-aFGF intervention,the treatment group postoperative edema (1.6±0.5 vs 2.2±0.5)、exudation (1.3±0.3 vs 1.8±0.4) 、erythema (1.6±0.3 vs 2.5±0.7)、burning pain assessment (1.7±0.6 vs 2.7±0.8)、skin elasticity increases (25.8±4.7% vs 5.2±2.2%) 、skin moisture increases (55.8±14.6% vs 27.6±6.8%)、pigment score (1.9±0.5 vs 2.7±0.5)、complete healing (4.2±1.6 vs 7.7 ±2.4) d.After treatment the treatment group and 6 patients had a spray treatment itching,itching control group 2 cases.Conclusion rh-aFGF joint Q-switched neodymium:yttrium-aluminum-garnet laser treatment of melasma,the laser can reduce postoperative complications,increase skin elasticity and moisture,reduce pigmentation occurs.
