摘要
目的探讨外周血清巨噬细胞抑制因子-1(MIC-1)检测在胃癌中的诊断价值。方法收集宁夏医科大学总医院2012年7月-2013年5月门诊及住院患者共132人,经胃镜检查及或手术组织病理检测诊断为慢性非萎缩性胃炎的患者34例、慢性萎缩性胃炎的患者33例、早期胃癌组(包括重度不典型增生)的患者24例,进展期胃癌的患者41例,使用ELISA实验法测定所有患者外周血清的MIC-1的浓度,并使用电化学发光法测定所有患者外周血清CEA、CA199的浓度。结果血清MIC-1浓度在胃癌(包括早期胃癌及进展期胃癌)明显升高,与慢性胃炎组(包括慢性非萎缩性胃炎及慢性萎缩性胃炎组)间的浓度差异有统计学意义(P均<0.01);血清MIC-1、CEA、CA199对早期胃癌的诊断灵敏度分别为79.2%、20.8%、25.0%,特异度分别为82.1%、97.0%、95.5%,准确度分别为81.3%、76.9%、76.9%;血清MIC-1、CEA、CA199对进展期胃癌的诊断灵敏度分别为82.9%、41.5%、43.9%,特异度分别为82.1%、97.0%、95.5%,准确度分别为82.4%、75.9%、76.9%。联合MIC-1、CEA、CA199在早期胃癌的诊断灵敏度为91.7%,特异度为80.6%,准确度为83.5%;联合MIC-1、CEA、CA199在进展期胃癌的诊断灵敏度为92.7%、特异度为80.6%、准确度为85.2%。结论血清MIC-1有望成为筛查胃癌尤其是早期胃癌的肿瘤标记物。
Objective To explore the diagnostic value of serum MIC - 1 for early gastric cancer. Methods 132 upper gastrointestinal patientsfiata of out- patient department and in -patient department in the General Hospital of Ningxia Medical University from July 2012 to May 2013 were collected. 34 of them were chronic non atrophic gastritis, 33 of them were chronic atrophic gastritis, 24 of them were at the early stage of gastric cancer (which included the severe atypical hyperplasia), while 41 patients were with advanced gastric canc- er. The concentration of serum MIC - 1 of all patients were determined by ELISA experiment and the concen- tration of CEA and CA199 were determined by ECL experiment. Results The concentration of serum MIC - 1 increased significantly in gastric cancer( including early gastric cancer and advanced gastric cancer) compared with chronic gastritis( including the nonatrophic gastritis and atrophic gastritis). The diagnostic sensi- tivety of serum MIC - 1, CEA, CA199 in early gastric cancer were 79.2% ,20.8% ,25.0% , and the diag- nostic specificity were 82. 1% , 97.0% , 95.5%. The diagnostic accuracy of them are 81.3% , 76.9% , 76.9%. The diagnostic sensitivety of serum MIC - 1, CEA, CA199 in advanced gastric cancer were 82.9%, 41.5% ,43.9% , and the diagnostic specificity were 82. 1% ,97.0% ,95.5%. The diagnostic accuracy of them were 82.4% ,75.9% ,76.9%. The diagnostic sensitivity and specificity of uniting serum MIC - 1, CEA, CA199 in early gastric cancer were 91.7% and 80.6% while the diagnostic accuracy was 83.5%. The diagnostic sensitivity and specificity of uniting serum MIC - 1, CEA, CA199 in advanced gastric cancer were 92.7% and 80.6% while the diagnostic accuracy was 85.2%. Conclusion MIC - 1 might be a new serum marker for screening gastric cancer especially for early gastric cancer.
