摘要
目的：研究Lichtenstein术式及前入路USH两种方法在无张力疝修补术患者中的临床效果及安全性。方法分析我院2012年4月~2013年4月收治的40例行无张力疝修补术患者，按照随机数字表法分为治疗组20例，给予Lichtenstein术式，对照组20例给予前入路USH。比较两组患者手术时间、术后排气时间、下床活动时间、恢复正常生活工作时间、住院时间以及术后并发症，随访1~2年，观察手术后复发率。结果（1）对照组手术时间显著低于治疗组，两组比较差异有统计学意义（P＜0.05），两组在术后排气时间、下床活动时间、恢复正常生活工作时间、住院时间方面比较（P＞0.05）。（2）对照组与治疗组并发症发生率分别30.0%（6/20），25.0%（5/20），两组比较（x2=0.125，P＞0.05）。（3）对照组与治疗组术后复发病率分别为20.0%（4/20），0（0/20），两组比较（x2=4.444，P＜0.05）。结论 Lichtenstein及USH两种方法在无张力疝修补术中各有优劣势，Lichtenstein可以减少手术时间，USH可以减少术后复发率，两种方法在并发症及手术各项指标方面比较无差异，可根据患者具体条件进行选择。
Objective To research the efficacy and safety of two kinds of methods of Lichtenstein operation and the former approach USH in patients with no tension hernia repair. Methods 40 cases who had tension hernia repair from April 2012 to April 2013 in our hospital were analyzed and divided into the treatment group（20 cases）,which were given the Lichtenstein operation, and the experience group（20 cases）, which were treated by anterior USH. The operation time, exhaust time, ambulation time, to resume normal life and work time, hospitalization time and postoperative complications of two groups were compared.The operation recurrence rates were observed during the follow-up of 1-2 years. Results（1）The control operation time was significantly lower than the treatment group（P 〈 0.05）, The exhaust time, postoperative ambulation time, to resume normal life and work time, length of hospital stay between the two groups were not significant（P 〉 0.05）.（2） The control group and the treatment group complication rates were 30%（6/20）, 25%（5/20）, the two groups had no statistical significance（x2=0.125, P 〉 0.05）.（3） The recurrence of disease rates of two groups were 20%（4/20）, 0（0/20）, the two groups was statistically significant（x2=4.444, P〈0.05）. Conclusion Lichtenstein and USH two methods have advantages and disadvantages of each in tension-free herniorrhaphy, Lichtenstein can reduce the operation time, USH can reduce the recurrence rate, two kinds of methods in the indicators complications and operation aspects has no difference, can choose according to the specific conditions of patients.
