摘要
目的比较观察各类型妊娠期高血压孕妇与正常妊娠孕妇凝血功能血小板指标,及血栓前状态(prethrombotic state,PTS)分子标志物水平。方法选择妊娠期高血压孕妇妊娠晚期76例,分为妊娠期高血压26例,轻度子痫前期组26例和重度子痫前期组24例;另外选择正常妊娠晚期孕妇(正常组)30例。所有病例均于入院后空腹抽取静脉血,分离血浆,采用酶联免疫吸附试验(ELISA)方法,检测凝血功能(凝血酶时间、活化部分凝血活酶时间、血浆凝血酶原时间及纤维蛋白原)、血小板指标(PLT、MPV、PDW),血栓前状态分子标志物D二聚体(D-II)、血管性血友病因子(vWF)水平。结果重度子痫前期组凝血功能、vwF水平均显著高于妊娠期高血压组及轻度子痫前期组(P<0.05),非常显著高于正常组(P<0.01)。妊娠期高血压组及轻度子痫前期组与正常组比较,差异无显著性(P>0.05)。结论重度子痫前期妊娠晚期患者血小板相关PTS分子标志物水平较正常妊娠晚期患者显著升高,其存在PTS。
Objective To compare the observation in patients with gestational hypertension and blood coagulation function in patients with normal pregnancy and platelet index ,molecular markers of prethrombosis state (PTS) .Methods Chosen late pregnancy in 76 patients with gestational hyperten-sion ,and divided into of gestational hypertension group for 26 cases ,mild preeclampsia group for 26 cases and severe preeclampsia group for 24 cases .Another for normal group with 30 cases of normal pregnancy late .All cases were fasting venous blood was drawn after admission ,the separation of plas-ma ,using enzyme-linked immunosorbent assay (ELISA) method ,in strict accordance with the kit in-structions ,testing blood coagulation function (thrombin time and activated clotting time live enzymes , plasma prothrombin time and fibrinogen) and platelet index (PLT ,MPV and PDW) ,blood clots be-fore state molecular markers D dimer (D-II) ,von willebrand factor (vWF) level .Results The blood coagulation function of severe preeclampsia group ,vwF levels were significantly higher than that of gestational hypertension group and mild preeclampsia group (P〈0 .05) ,significantly higher than in normal group (P〈0 .01) .Gestational hypertension and mild preeclampsia group compared with nor-mal group ,there were no statistically significant differences (P〉0 .05) .Conclusion Patients with se-vere preeclampsia in late pregnancy platelet related PTS molecular markers level is significantly in-creased in patients with normal late pregnancy ,the existence of PTS .And with the gestational hyper-tension disease ,PTS increased risk .
