摘要
目的分析鼻咽癌放疗患者合并口腔真菌感染的临床特点及治疗效果,为临床治疗提供参考依据。方法选取2009年3月-2012年12月鼻咽癌患者共112例;观察其口腔真菌的感染率,分析发生口腔真菌感染的危险因素以及治疗效果,数据采用SPSS13.0进行分析。结果进行放疗或者同期进行放化疗的鼻咽癌患者发生口腔感染率74.11%,单纯放疗患者口腔真菌感染率72.15%,同期进行放化疗患者的感染率为78.79%;口腔真菌感染患者口腔pH值<6时感染率86.75%,住院时间>40d患者的感染率有69.88%,使用≥2种抗菌药物感染患者感染率为73.49%。结论鼻咽癌患者在接受放疗时应注意口腔清洁卫生,采用碳酸氢钠进行漱口,保持口腔pH>6,避免并发口腔真菌感染。
OBJECTIVE To analyze the clinical effect and characters on oral fungal infections for the patients with nasopharyngeal carcinoma under the treatment of radiotherapy,so as to provide reference to clinical treatment.METHODS A total of 112 cases of nasopharyngeal carcinoma in our hospital from Mar.2011 to Mar.2012 were selected.Selected patients were observed the ratio of oral fungal infections and the risk factors and treatment effect of fungal infections.RESULTS The occurrence of oral infections were more than 74.11% for the nasopharyngeal carcinoma patients with radiotherapy or concurrent chemoradiotherapy,and the fungal infection rate was 72.15% of patients with only radiotherapy,78.79% of infection rate only with chemoradiotherapy.Fungal infection patients with oral pH 6 had infection ratio of 86.75%,and comparing to negative cases the difference was significant;there were 69.88% of infection patients with hospitalization time40 days.In antibiotic use,antibiotics the incidence of infections was 73.49% in the patients who used no less than two antibiotics.CONCLUSION NPC patients receiving radiotherapy should pay attention to oral hygiene,mouthwash sodium bicarbonate can be used to maintain oral pH above 6,avoid concurrent oral fungal infections.
