白细胞及降钙素原在慢性阻塞性肺疾病患者感染的相关性分析 被引量:18

摘要 目的探讨白细胞和降钙素原在慢性阻塞性肺疾病(COPD)伴感染中的相关性,为临床诊治提供参考。方法选取2011年8月一2013年8月收治的COPD急性加重(AECOPD)患者79例,检测所有患者的降钙素原、白细胞分类计数,根据降钙素原的检测结果将患者分为A组(降钙素原≥0.5ng/m1)12例、B组(降钙素原0.1~0.5ng/m1)21例和C组(降钙素原<0.1ng/m1)46例;对比3组患者的降钙素原、白细胞计数和中性粒细胞的水平,并分析3个指标之间的相关性,采用SPSS17.0进行分析。结果3组降钙素原和白细胞分类计数均无显著的相关性;A组患者的白细胞计数和中性粒细胞比例最高,达到(15.47±8.96)×10^9/L和(84.55±10.36)%,B组次之,C组最低;降钙素原水平升高,白细胞分类计数水平也升高,但两者之间的变化趋势没有必然性。结论白细胞分类计数和降钙素原在COPD伴感染的诊断中无相关性,两者联合检测有助于早期诊断细菌感染,规范临床用药。 OBJECTIVE To investigate the correlation between leukocyte count and procalcitonin in the chronic obstructive pulmonary disease (COPD) patients complicated with infections so as to provide a reference for clinical diagnosis and treatment. METHODS Totally 79 acute exacerbated COPD (AECOPD) patients in the hospital during Aug. 2011 to Aug. 2013 were selected, the procalcitonin, leukocyte count were detected for all the patients. According the procalcitonin test results, the patients were divided into group A (procalcitonin ≥0.5 ng/ mL, n=12), group B (0. 1ng/mL ≤ procalcitonin 〈0.5 ng/mL, n=21), and group C (calcitonin plasminogen 〈0.1 ng/mL, n:46). The above indexes were compared between the three groups, and the correlation analysis was performed as well. Data were analyzed with SPSS 17.0. RESULTS No significant correlation was observed for procalcitonin and leukocyte count in the three groups. White blood cell count and neutrophil showed the highest proportion in group A, which were respectively (15.47 ± 8. 96) ×10^9/L and (84. 55±10.36) %, followed by group B, and the lowest in group C. When the concentration of procalcitonin increased, the leukocyte count increased as well, but there was no inevitable relationship between the change trends of the two indexes. CONCLUSION No correlation can be found between leukocyte count and procalcitonin in the diagnosis of infection in COPD. The combined detection is helpful for early diagnosis of bacterial infections as wen as the reasonable use of antibiotics.
