摘要
目的探讨儿童上气道咳嗽综合征的临床特点与对症治疗。方法收集2012年1月至2013年12月耳鼻咽喉科及儿科诊断为UACS的儿童患者,获得完整临床资料及完成随访病例206例。分别记录主要症状和体征,辅助检查结果 (影像学检查、变应原血检测、电子鼻咽喉镜检查、血常规检查),治疗前后咳嗽积分。结果日间频繁咳嗽、鼻塞流涕、咽痒、鼻痒是其主要症状,63.6%患者可见咽后壁鹅卵石样滤泡,37.9%的患者咽后壁可见分泌物附着。合并有慢性鼻窦炎68例,明确有变应性因素影响83例。慢性鼻-鼻窦炎、变应性鼻炎、慢性咽炎是引发儿童UACS主要的三大疾病。治疗后按照咳嗽积分疗效评定总有效率为86.9%。结论上气道咳嗽综合征好发于学龄前期及学龄期儿童咳嗽,分析儿童UACS的临床特点可减少误诊,提高治愈率。
Objective To discuss the clinical features and symptomatic treatment of upper airway cough syndrome （UACS） in children. Methods 206 cases of children with UACS in our hospital from January 2012 to December 2013 were selected in this study. The main symptoms and signs, the cough score before and after the treatment, and results of laboratory examinations （imaging examination, blood detection of allergen, electronic nasopharyngoscope examination, and blood routine examination） were recorded. Results Daytime frequent cough, nasal congestion and runny nose, throat itching, nasal itching were the main symptoms. Pharyngeal cobblestone follicles were found in 63.6%of patients, and pharyngeal secretions visible attachment was found in 37.9%of patients. 68 patients were complicated with chronic sinusitis, and 83 patients had clear allergic factors. Chronic sinusitis, allergic rhinitis, and chronic pharyngitis were three major diseases that caused UACS in children. The cough score showed that the total effective rate was 86.9%. Conclusions Upper airway cough syndrome mostly occurs in children at preschool and school-age. Analysis of clinical characteristics of children with UACS can reduce misdiagnosis, and improve the cure rate.
