彩色多普勒超声和钼靶X线诊断小乳腺癌的临床价值比较 被引量:7

摘要 目的:比较彩色多普勒超声、钼靶X线对小乳腺癌的诊断价值。方法:回顾分析经手术病理证实的92例小乳腺癌患者(114个病灶)的彩色多普勒超声和钼靶X线资料特征,比较二者分别诊断小乳腺癌的灵敏度及特异性。结果:超声检查小乳腺癌的灵敏度为86.18%,特异性为64.10%,假阴性率为4.52%;钼靶X线检查小乳腺癌的灵敏度为82.25%,特异性为90.37%,假阴性率为8.97%,二者之间的差异具有统计学意义(P<0.05)。钼靶X线对微小钙化的显示优于超声(P﹤0.05),钼靶X线对多灶性小乳癌的检出率明显高于超声。结论:钼靶X线及彩色多普勒超声在发现和诊断小乳腺癌上均具有重要价值,两者结合可进一步提高小乳腺癌的检出率和诊断的准确率。 Objective: To compare the effectiveness of the color Doppler uhrasonography and Mammography in the diagnosis of small breast cancer, and to assess the value of these modalities. Methods A total of 92cases ( 114 lesions) diagnosed as small breast cancer by surgery and pathology were both received uhrasonography and Mammography examinations. The imaging features of small breast cancer in the two imaging methods and the sensitivity and specificity of the two methods were analyzed. Results The sen- sitivity, specificity, false negative rate were 86. 18%, 64. 10%, 4. 52% for ultrasonography and 82. 25%, 90. 37%, 8. 97% for mammography, respectively. The differences between the uhrasonography and mammography were statistically significant ( P 〈 0. 05 ) . Mammography was more sensitive for microcalcifications in small breast cancer than uhrasonography ( P 〈 0. 05 ) . Mammography was more sensitive for detecting multi - lesions and more accurate for extension of the lesion. Conclusion It is of high clinical value for mammography and high - frequency color ultrasound in the diagnosis of small breast cancer, the combination of the two can further improve the detection and the diagnostic accuracy rate of small breast cancer.
