摘要
目的探讨孕妇血清IgG亚类检测对新生儿ABO溶血性疾病的早期诊断价值。方法纳入2011年8月至2013年6月于本院进行定期产检并分娩的血型为O型(丈夫为非O型血)的孕妇。将发生HDN的孕妇30例作为试验组,同时选取同期检测但并未发生HDN的孕妇50例作为对照组。对孕妇IgG抗A(B)效价和新生儿ABO溶血的关联性进行分析,对试验组及对照组间IgG亚类含量进行比较。结果且新生儿ABO溶血疾病发病率随着IgG抗A(B)效价水平升高而上升,差异具有显著性(P<0.05);试验组孕妇与患儿IgG1、IgG3、IgG4组间比较均差异显著(P<0.05);对照组孕妇与试验组孕妇IgG2差异显著(P<0.05);对照组孕妇IgG1、IgG3、IgG4组间差异均显著(P<0.05)。结论新生儿ABO溶血发病随母体IgG抗体效价水平升高而上升,对于夫妻血型不合的O型血孕妇应提早进行IgG抗体效价水平和IgG亚类含量的检测,以早期发现、诊断和治疗,改善预后。
Objective Objective To investigate the early diagnostic value of serum IgG subclasses testing on ABO hemolytic disease of the newbom(HDN). Methods Pregnant women with blood type O (husband with non O blood type) in our hospital from August 2011 to June 2013 for regular check ups and delivery were included. 30 cases of pregnant women with HDN after delivery were set as the experimental group and women without HDN at the same time as the control group.Relevance of IgG anti A (B) titer of pregnant women with ABO haemolytic disease of the newborn was analyzed and IgG subclass content between the experimental group and the control group was compared. Results The incidence of neonatal ABO hemolytic disease increased with IgG anti A (B) titres increased, and the difference was significant (P 〈0.05); IgG1, IgG3, IgG4 of .pregnant women and children in the experimental group were compared and there were statistically significant difference between groups (P 〈0.05). IgG2 of pregnant women in the control group and experimental group had significant difference (P 〈0.05); and there were significantly differences in IgG1, IgG3, IgG4 groups of pregnant women in control group(P 〈0.05). Conclusions The incidence of neonatal ABO hemolytic disease increased with the increase of IgG antibody titers. Pregnant women with O blood type contributed to incompatible blood type between couples should be detected early in IgG antibody titer and IgG subclass content for early detection, diagnosis and treatment, and prognosis improvement.
