ICU中不同时段BiPAP呼吸机治疗Ⅱ型呼吸衰竭患者的临床疗效评价 被引量:10

摘要 目的分析并评价重症监护病房中不同时段BiPAP呼吸机治疗Ⅱ型呼吸衰竭患者的临床疗效。方法选择湘潭县人民医院2011年1月至2012年12月收治的呼吸衰竭患者44例,均给予常规药物实施治疗,根据应用呼吸机时间不同分为两组:两组均应用BiPAP呼吸机治疗,对照组22例白天使用呼吸机,观察组22例晚间使用呼吸机。对全部患者实施各项监测及不同时段的动脉血进行血气分析。结果观察组在治疗后各动脉血血气分析指标(pH、二氧化碳分压及血氧分压)与各生命体征指标(心率、呼吸频率及平均动脉压)均显著优于对照组(P<0.05)。观察组应用BiPAP呼吸机实施治疗并顺利脱机者21例(70%),对照组9例(30%)(P<0.01);观察组病情未见明显改善或恶化并实施气管插管或气管切开者3例(21.4%),对照组11例(78.6%)(P<0.01)。结论晚间持续应用BiPAP呼吸机辅助呼吸对重症监护病房Ⅱ型呼吸衰竭患者具有明显的临床疗效,能够有效改善患者的呼吸功能,提倡临床广泛推广。 Objective To analyze the clinical curative effect BiPAP ventilation for type I1 respiratory failure at different periods in ICU. Methods A total of 44 cases of respiratory failure patients admitted by Xiangtan People's Hospital during Jan. 2011 and Dec. 2012 were selected, all of which were given conven- tional medication treatment, divided into observation group and control group according to the random-number table method ,22 cases in the control group used ventilator during daytime ;22 cases in the observation group used ventilator during the night time. Blood gas analysis of arterial blood and various indexes monitoring of all patients were done at different periods. Results All the 44 patients received BiPAP ventilator treatment,the arterial blood gas analysis indexes( pH,PaCO2 and PaO2 ) and the vital signs( HR,RR and MAP) of the observation group were significantly better than the control group(P 〈0.05). There were 21 cases(70% ) of successful de-BiPAP ventilation in the observation group, and 9 (30%) in the control group ( P 〈 0. 01 ) ; there were 3 cases( 21.4% ) which did not improve or deteriorated significantly and were given endotracheal intabation or tracheotomy in the observation group, 11 cases (78.6%) in the control group ( P 〈 0.01 ). Conclusion The continuous application of BiPAP ventilator during nighttime has obvious curative effect on ICU patients with type Ⅱ respiratory failure,which can effectively improve the patient's respiratory function, and is suggested to be widely promoted in clinical.
