合并房室阻滞的房室结折返性心动过速的射频消融治疗

摘要 目的 分析术前合并房室阻滞的房室结折返性心动过速(AVNRT)进行射频消融(RFCA)治疗的方法与结果。方法 回顾分析本院近2年来术前存在房室阻滞的AVNRT患者行RFCA治疗的情况及随访结果。结果13例患者[男8例,年龄(64±14)岁]中4例曾行RFCA治疗,8例为原发性PR间期延长,1例曾行房间隔修补术,其中诊断慢快型AVNRT 10例,快慢型和慢慢型各1例,未诱发心动过速1例。未诱发者未行消融,其余所有患者消融均成功,随访5-24个月期间未见心动过速复发及房室阻滞加重。结论 在合并房室阻滞的AVNRT患者进行消融时应根据心内电生理检查情况决定消融策略。 Objective To analyze the method and result of the radiofrequency catheter ablation (RFCA) in the patients (pts) with atrioventricular node reentrant tachycardia (AVNRT)and pre-existing atrioventricular (AV) block. Method Retro-analyze the 13 patients with AVNRT and AV block that underwent the RFCA in the last 3 years. Results Four pts had underwent RFCA, 8 pts were primary PR prolong and 1 pt had underwent atrial septal repaired. Ten pts were diagnosed slow-fast AVNRT and slow-slow ( 1 pt) and fast-slow AVNRT ( 1 pt). No tachycardia was induced in 1 pt. Conclusions The ablation strategy should be decided by the results of EP study of every patient.
