青蒿素对支气管哮喘外周血Treg/Th17的调控机制 被引量:6

In vitro study in the regulation of artemisinin on bronchial asthma of peripheral blood Th17/Treg

摘要 目的探讨青蒿素对支气管哮喘患者外周血Treg/Th17表达的调控机制。方法将随访的145例哮喘患者按照ACT评分分成完全控制组、部分控制组、未控制组,再从每组中按性别、年龄、BMI匹配随机分别抽取20例、21例、7例作为研究对象,入选同期于医院就诊的哮喘急性发作期患者18例和医院体检的健康成人19例作为对照组。流式细胞术检测各组CD4+T淋巴细胞Treg和Th17的表达,将CD4+T淋巴细胞用一定浓度的青蒿素干预共培养24 h检测对比。结果未控制组及急性发作期哮喘患者外周血CD4+T淋巴细胞Th17比例明显高于正常对照组、完全控制组及部分控制组哮喘患者(P<0.05),Treg比例明显低于正常对照组、完全控制组及部分控制组(P<0.05);完全控制组及部分控制组哮喘患者外周血CD4+T淋巴细胞Treg比例稍低于正常对照组(P<0.05),而Th17比例较正常组差异无统计学意义(P>0.05);与青蒿素共浴培养24 h后,未控制组哮喘患者CD4+T淋巴细胞中Th17较干预前明显减低(P<0.05),Treg较干预前明显升高(P<0.05);急性发作组哮喘患者Th17较干预前明显减低(P<0.05),Treg较干预前明显升高(P<0.05);部分控制组哮喘患者Th17较干预前差异无统计学意义(P>0.05),Treg较干预前差异无统计学意义(P>0.05);完全控制组哮喘患者Th17较干预前稍减低(P<0.05),Treg较干预前差异无统计学意义(P>0.05)。结论青蒿素在体外对支气管哮喘,尤其是难治性哮喘的外周血CD4+T淋巴细胞中Treg/Th17有一定的调控作用,并在一定程度上可改善难治性哮喘患者CD4+T淋巴细胞中Treg/Th17免疫失衡。 Objective To detectin vitrostudy in the regulation of artemisinin on bronchial asthma of peripheral blood Th17/Treg. MethodsAccording to the asthma control test (ACT) scores, followed-up 145 patients with asthma were divided into controlled, partly controlled and uncontrolled group, then which were selected randomly 20, 21, and 7 cases as the study ones by sex, age and BMI matching, meanwhile 18 patients with acute exacerbation of asthma and 19 healthy adults as the control. Artemisinin and CD4+T lymphocytes were co-cultured for 24 hours, used flow cytometry to detect the expression of Th17 and Treg and compared with no co-cultured. ResultsTh17 ratio in CD4+T lymphocytes of the peripheral blood in uncontrolled group and acute exacerbation of asthma was significantly higher than that of other groups(P〈0.05), and Treg ratio in which was significantly lower than that of other groups(P〈0.05). Meanwhile, Treg ratio in other asthmatic groups was slightly lower than that of normal group (P〈0.05). After Artemisinin and CD4+T lymphocytes were co-cultured for 24 hours, Th17 ratio in uncontrolled group and acute exacerbation of asthma was significantly decreased (P〈0.05), and Treg ratio was significantly increased (P〈0.05). The ratios of Th17 and Treg in partly controlled group were not different. Th17 ratio in controlled group was slightly decreased than before (P〈0.05), but Treg ratio was not different.ConclusionsIn vitro, artemisinin has a certain regulation function on bronchial asthma of peripheral blood Th17/Treg, especially in refractory asthma. Artemisinin can improve in some degree the imbalance of TH17/Treg in CD4+ lymphocytes in patients with refractory asthma.
