摘要
目的探讨肝硬化肝脏流入道血流动力学变化和肝功能Child-Pugh分级的相关性。方法选取肝硬化患者62例,依据Child-Pugh肝功能分级不同分为A、B、C 3组,应用超声多普勒技术对2组肝动脉血流量(QHA)、搏动指数(PI),门静脉最大血流速度(Vmax)、平均血流速度(Vmea)、阻力指数(RI)、血流量(QPV)进行测定,并对肝脏流入道血流动力学变化与不同肝病肝功能的相关性进行综合分析。结果 A、B、C 3组的RI、PI相比较,B组肝硬化患者明显较A组高。肝脏流入道血流动力学改变在A、B 2组差异具有统计学意义(P<0.05),同时病变患者与健康参照组之间差异也有统计学意义(P<0.05)。肝固有动脉血流速度、血流量与肝病变程度呈正相关,随着Child-Pugh肝功能分级的加重而降低,门静脉内径、血流量随Child-Pugh肝功能分级的加重逐渐增高。结论肝硬化肝脏整体流入道血流动力学的改变与Child-Pugh肝功能分级有较好的相关性,肝动脉血流的异常改变可为肝硬化诊断提供依据,对肝硬化早期诊断、预防及治疗起积极作用。
Objective To investigate the relevance of inflow tract hemodynamic change and Child-Pugh grading of liver function in cirrhosis. Methods A total of 62 patients with liver cirrhosis were divided into groups A, B, C on the basis of Child-Pugh grades, ultrasonic doppler technology was applied to detect the hepatic blood flow (QH.A), pulse index (PI); Portal venous blood flow velocity (Vmax), the largest average blood flow velocity (Vmea), resistance index (RI), and blood flow (QPV) were detected and these parameters were formed into a comprehensive analysis of correlation between inflow tract hemodynamic change and liver function. Results RI and PI of three groups were compared. RI and PI in Group B were significantly higher than those of group A. The inflow tract hemodynamic changes showed a significant difference between groups A and B ( P 〈 0.05), and pathological changes of differences between patients and healthy control group have also statistical significance ( P 〈 0.05). Proper hepatic artery blood flow velocity, blood flow and illness degree were positively associated, declining with the increase of Child-Pugh grades. Portal vein diameter and blood flow reduced with the deterioration of Child-Pugh grades gradually. Conclusion Hemodynamic changes of inflow tract and Child-Pugh grades have good correlation. The abnormal changes of hepatic artery blood flow can provide basis for the early diagnosis of cirrhosis. They are effective for early diagnosis, prevention and treatment of cirrhosis.
