摘要
目的 探讨体外循环术后高浓度静脉补钾治疗低钾血症的临床护理经验.方法 回顾性分析我院2011年12月至2012年10月体外循环术后96例心脏病人的临床资料.浓度为1% ~3%的氯化钾溶液,以6.7~16.8 mmol/h的速度,通过中心静脉输注,以纠正低钾血症.结果 62例病人从中心静脉进行高浓度静脉补钾后,血清钾上升明显,基本维持在4.2~ 4.5 mmol/L,25例补钾2h后复测血清钾上升不明显,同时给予补充镁离子,积极纠正碱中毒后,血清钾上升到4.2 mmol/L以上的理想水平,未出现高钾血症.采用此种方法能安全有效提高血钾水平,未发现致命性心律失常.结论 高浓度的氯化钾溶液经中心静脉输注,能够快速、安全的提高其血钾水平,及时预防和纠正低血钾的发生,减少术后各种与低血钾有关的并发症的发生.
Objective To investigate the clinical nursing experience after cardiopulmonary bypass in high concentration potassium treatment of hypokalemia. Methods Retrospective analysis between December 2011 and October 2012 extracorporeal circulation the clinical data of 96 cases of postoperative cardiac patients for 1 ~ 3% potassium solution concentration, with tendency of 6.7 ~ 16.8 mmol/h speed,through the central venous infusion,to rectify the hypokalemia. Results To high concentrations of 62 patients from the center vein after intravenous potassium supplement, serum potassium increased significantly, the basic tendency in 4.2 ~ 4.5 mmol/L, 25 cases of repetition measurement of serum potassium increased after the potassium supplement 2 h is not obvious, at the same time give supplemental magnesium ions, positive correction after alkalosis, the serum potassium rises to more than 4.2 mmol/L levels. Con- clusion High concentrations of potassium chloride solution by central venous infusion, can safely and quickly improve the level of the potassium,timely prevention and correction of the occurrence of hypokalemia, reduce the postoperative complications associated with hypokalemia.
