摘要
目的观察下肢复合伤中应用高压氧的效果。方法 82例下肢开放性复合伤患者随机分为两组,均采用手术、清创、皮瓣转移,术后抗生素预防感染的常规治疗,观察组42例在常规治疗基础上加用高压氧治疗,共进行20 d。观察组治疗前后两组患者肌肉组织恢复情况、皮瓣转移存活率和骨折的愈合情况。结果观察组在治疗后局部肿胀消退,无疼痛或轻度压痛患者27例(64.3%),对照组在治疗后局部肿胀消退,无疼痛或轻度压痛患者13例(32.5%),观察组明显优于对照组(P<0.05)。观察组有21例患者行皮瓣转移,存活20例(95.2%),对照组有18例患者行皮瓣转移,存活15例(83.3%),观察组皮瓣转移存活率明显高于对照组(P<0.05)。平均随访12个月,观察组骨折平均愈合时间为4个月,愈合41例,愈合率97.6%;对照组骨折平均愈合时间为6个月,愈合37例,愈合率92.5%.观察组在愈合时间方面明显少于对照组(P<0.05),观察组骨折愈合率明显高于对照组(P<0.05)。结论高压氧治疗可以减轻下肢复合伤炎性反应、提高皮瓣转移存活率和加快骨折愈合。
Objective To study effectiveness of hyperbaric oxygen in patients with lower limb composite injuries.Methods 82 cases of open lower limb injuries were randomly divided into two groups complex, both conventional treatment of surgical debridement, flap, and postoperative antibiotics to prevent infection.The observation group(n=42 cases) were treated through hyperbaric oxygen therapy plus conventional therapy.They were treated for 20 days. Before and after treatment, muscle tissue recovery, healing skin flap survival and fractures in two groups were observed.Results In the observation group after treatment, 27 cases (64.3%) patients had swelling subsided, no pain or mild.In the control group had 13 patients (32.5%).The observation group was significantly superior than in the control group (P〈 0.05).In the bservation group, 21 patients underwent skin flap survival for 20 patients (95.2%) and in the control group, 18 patients underwent skin flap survival for 15 cases (83.3%) and the observation group was significantly higher flap survival (P〈 0.05). The average follow-up of 12 months, the average healing time of fractures of the observation group was 4 months, 41 cases of healing, the healing rate was 97.6%.The control group average healing time was 6 months, 37 cases of healing, the healing rate was 92.5%in the observation group healing timing of significantly less than the control group (P〈 0.05), the observation group was significantly higher healing rate (P〈 0.05).Conclusions Hyperbaric oxygen therapy can reduce lower extremity injuries complex inflammatory response, enhance skin flap survival and accelerate fracture healing.
