摘要
目的探讨肾小球滤过率(GFR)与缺血性脑血管病患者动脉狭窄的关系。方法收集199例缺血性脑血管疾病患者的临床资料,所有患者均进行数字减影血管造影术(DSA),根据动脉狭窄支数及程度(狭窄率≥50%)分为对照组(51例)、单支狭窄组(49例)、两支狭窄组(42例)、多支狭窄组(57例)。根据GFR三分位数对患者进行分组,<83.93 m L/(min·1.73m2)为低水平组(66例)、83.93~101.41 m L/(min·1.73m2)为中水平组(67例)、≥101.42 m L/(min·1.73m2)为高水平组(66例)。分析患者临床资料及GFR与缺血性脑血管病患者动脉狭窄之间是否存在相关性。结果与对照组比较,双支狭窄组和多支狭窄组随着狭窄支数的增加,而GFR逐渐降低(P<0.05)。低水平组中,两支狭窄组、多支狭窄组例数较对照组、单支狭窄组多(P<0.05);中水平组中差异无统计学意义(P>0.05);高水平组中,对照组、单支狭窄组例数较两支狭窄组、多支狭窄组多(P<0.05)。Logistic回归分析显示GFR是缺血性脑血管病动脉狭窄的危险性因素(OR=0.982,P=0.034)。结论 GFR水平与缺血性脑血管病动脉狭窄程度密切相关,是有效识别缺血性脑血管病动脉狭窄程度的临床指标。
Objective To discuss the association between glomerular filtration rate( GFR) and artery stenosis in patients with ischemic cerebrovascular disease. Methods The clinical data of 199 patients with ischemic cerebrovascular disease who underwent digital subtraction angiography( DSA) were collected,and the patients were divided into four groups according to the findings of cerebral angiography: 51 cases of control had normal artery stenosis,49 cases of single artery stenosis more than 50%,42 cases of double artery stenosis more than 50%,57 cases of multiple artery stenosis more than 50%. The subjects were divied into three groups according to GFR level in tertile: <83. 93 m L /( min·1. 73 m2) as low level group( 66 cases),83. 93 ~ 101. 41 m L/( min·1. 73 m2) as horizontal level group( 67 cases),≥101. 42 m L /( min·1. 73 m2) as high level group( 66 cases). The relationship between GFR level and artery stenosis was explored.Results Compared with control group,the more artery stenosis was the lower level of GFR apart from single artery stenosis group. The differences had statistical significances( P < 0. 05). In the low level GFR group,the double and multiple group proportions were higher than the control group and single group( P < 0. 05). In the horizontal level of GFR,the differences among every group had no statistical significance( P > 0. 05). In the high level GFR group,the proportions of control group and single group were higher than those of the double and multiple group( P < 0. 05). Logistic regression analysis showed that the reduced GFR was one of the independent risk factors of artery stenosis in patients with ischemic cerebrovascular disease( OR = 0. 982,P = 0. 034). Conclusion The reduced GFR correlates with the severity of artery stenosis in patients with ischemic cerebrovascular disease. This may has potential clinical value as markers for the identification of artery stenosis in patients with ischemic cerebrovascular disease.
