摘要
目的评价调强放疗联合重组人p53腺病毒注射液治疗中晚期巨块型宫颈癌的临床疗效及安全性。方法 20例确诊的中晚期巨块型宫颈癌患者随机分为治疗组(调强放疗结合p53基因治疗)10例和对照组(单纯调强放疗)10例,治疗组每周五给予重组人p53腺病毒注射液瘤内注射1次,共4~5次,两组患者放疗剂量和放疗方法相同,治疗结束后2个月复查盆腔CT或MRI以观察肿瘤变化。结果治疗组10例,CR 6例(60.0%)、PR 3例(30.0%),有效率90.0%;对照组10例,CR 2例(20.0%)、PR 1例(10.0%),有效率30.0%,其中治疗组有效率明显高于对照组(90.0%vs 30.0%,双侧P=0.020,单侧P=0.010)。毒副作用治疗组除9例一过性低热、10例注射部位一过性疼痛外,余毒副反应两组相似。结论调强放疗联合重组人p53腺病毒注射液治疗中晚期宫颈癌是安全有效的,而且相较于单纯调强放疗具有较好疗效。
Objective To evaluate the clinical effects and safety of intensity modulated radiation therapy( IMRT) combined with recombinant human p53 adenovirus injection in the treatment of middle-late type giant piece of cervical cancer. Methods Twenty patients with middle-late type giant piece of cervical carcinoma were divided randomly into the treatment group( IMRT combined with p53 gendicine injection) and the control group( single IMRT),with 10 cases in each group. Recombinant human p53 adenovirus intratumor injection was performed 1 time on every Friday,with a total of 4 to 5 times. Radiotherapy plan of two groups was in agreement. Two months after treatment,CT or MRI was reviewed to observe the change of tumor volume. Results In the treatment group,CR was in 6 patients( 60. 0%),PR 3 patients( 30. 0%),and the effective rate was 90. 0%; in the control group,CR was in 2 patients( 20. 0%),PR 1 patient( 10. 0%),and the effective rate was 30. 0%. The effective rate of treatment group was higher than that of the control group( 90. 0% vs 30. 0%,Exact Sig.( 2-sided) P =0. 020,Exact Sig.( 1-sided) P = 0. 010). Nine patients presented transient low fever and ten patients transient pain in injection region; the other side effects in thr two groups were the same. Conclusion The IMRT combined with recombinant human p53 adenovirus injection in the treatment of middle-late type giant piece of cervical cancer is safe with better curative effect compared with single IMRT.
