摘要
目的 探讨玻璃体手术术后视网膜脱离复发的主要原因。方法 回顾分析本科 1998年 7月～ 2 0 0 1年 7月视网膜脱离术后复发病例共 39例 (39只眼 )。男性 2 6例 ,占 6 6 .6 7% ;女性 13例 ,占 33.33%。年龄从 13岁～ 6 8岁 ,平均 49.33岁。复发时间从 0 .5月到 2 6月 ,平均 4.6 6月。右眼 2 3只 ,左眼 16只。玻璃体手术前赵氏膜分级 : 级膜 18只眼 , A 膜 14只眼 , B1 膜 4只眼 , B2 膜 2只眼 , B3 膜 1只眼。复发后分别采用巩膜外冷凝环扎加压手术、玻璃体剥膜手术、气液交换手术眼内注入 C3 F8、气油交换手术。结果 视网膜脱离复发后经检查发现视网膜皱襞形成和前部 PVR导致再脱离的有 16只眼 ,因出现新孔导致视网膜再脱离的有 15只眼 ,因原孔未封闭导致再次脱离的有 8只眼。 PVR导致再脱离的病例平均复发时间为 7.13月 ,出现新孔的病例平均复发时间为 2 .0 9月 ,原孔未封闭导致再次脱离的病例平均复发时间为 1.96月。结论 视网膜脱离术后复发多因视网膜前膜增生。
Objective To investigate the main cause of recurrent retinal detachment after vitrectomy.Methods Retrospective analysis the clinical data of 39 cases (39 eyes) of recurrent retinal detachment after vitrectomy.There are 26 male cases and 13 female cases.The age is from 13 to 68 with average 49.33.The recurrent time is from 0.5 months to 26 months with average 4.66 months.23 eyes are in right and 16 eyes are in left.According to the Zhao's membrane formation classification,there are 18 eyes in grade Ⅱ,14 eyes in grade Ⅲ A,4 eyes in grade Ⅲ B1 ,2 eyes in grade Ⅲ B2 ,1 eye in Ⅲ B3 before primary vitrectomy.Scleral buckling surgery with cryopexy,vitreoretinal surgery,gas-liquid exchange surgery with C 3F 8 tamponade or the surgery of silicone oil injection are selected to take after the recurrence.Results 16 eyes found to redetach are due to the retinal folds formation and anterior PVR.15 eyes are due to new retinal holes.8 eyes are due to original holes reopened.The average recurrent time to PVR is 7.13 months,due to new holes is 2.09 months,due to re-opened original holes is 1.96 months.Conclusion The results suggested that the main causes of the recurrent retinal detachment after vitrectomy are preretinal membrane proliferation,anterior PVR and the opening holes.
