摘要
目的 探讨热休克蛋白 71(HSP71)抗体在感音神经性聋患者体内存在的临床意义。方法 对 2 9例原因不明的感音神经性聋患者进行试验性激素治疗 ;以纯化的人类重组热休克蛋白 71为抗原 ,用改良免疫转印迹技术 (Westernblot)检测患者血液中热休克蛋白 71抗体 ,分析HSP71抗体与激素治疗反应之间的关系。结果 2 9例中有 13例对激素治疗有效 ,其中 11例HSP71抗体阳性 ,2例阴性 ;16例无效病例中有 3例抗体阳性 ,13例阴性 ,二组之间抗体阳性率有显著性差异 (P <0 .0 1) ;14例HSP71抗体阳性患者中有 11例对激素治疗效果好 ;而抗体阴性的 15例患者中仅有 2例对激素治疗有反应。在预测激素治疗反应方面HSP71抗体的敏感性和特异性分别为 78.5 7%、86 .6 7% ,反应阳性预测值为 84.6 1%。结论 HSP71抗体在预测激素治疗反应方面具有很高的敏感性和特异性 ;
Objective To explore the clinical significance of autoantibody to heat shock protein 71 in sensorineural hearing loss.Methods A prospective study of patients with sensorineural hearing loss of unknown etiology was conducted.Plasma autoantibody to heat shock proteins 71(HSP71) was detected by Western blot using purified recombinant human HSP71 as antigen.Relationship of Western blot assay and response to corticosteroid therapy in patients was analyzed.Results Of 29 patients,13 had a positive response to corticosteroid therapy,among them, 11 cases were antibody-positive.In the corticosteroid nonresponders,there were only three patients who were antibody-positive(P<0.01).In the 14 cases with antibody-positive,11 had a positive response to corticosteroid therapy,while there were only 2 corticosteroid responders in patient who were antibody-negative.In predicting corticosteroid responsiveness,the sensitivity and specificity of the Western blot for HSP71 were 78.57%?86.67% respectively.Conclusion Autoantibody to HSP71 might serve as a specific marker for a corticosteroid responsiveness in sensorineural hearing loss of unknown etiology.In patient with antibody-positive,a trial of corticosteroid therapy could be given with good conviction because the test was quite specific.
