摘要
目的 评价术前超声检查对预测胆囊炎伴胆囊结石的患者行腹腔镜胆囊切除术 (L C)难易程度的应用价值 .方法 收集河南武警总队医院及西京医院 1999- 0 3/1999- 0 8的2 4 0例行 L C住院患者的资料 .术前超声声像图检查对胆囊轮廓、囊壁厚度、囊壁回声、囊内回声、胆囊颈部显示情况进行综合分析 ,评估胆囊与周围组织的粘连程度 ,并与手术进行对照 .结果 超声检查综合性评估胆囊与周围组织粘连程度的灵敏度为 73% ,特异度 98% ,准确度 96 % .结论 术前超声检查对 L
AIM To evaluate the application of preoperative ultrasonography to the prediction of the difficulty of laparoscopic cholecystectomy (LC) in patients of cholecystitis accompanied by gallstone. METHODS We prospectively analyzed the sonographic findings in 240 cases of cholecystitis accompanied by gallstone admitted to our hospital from March to August, 1999,which were confirmed by ultrasonography.Before LC, the gallbladders were comprehensively examined to observe the contour, wall thickness, wall echoes, intracyst echoes, gallbladder necks in these patients, Preoperative data and intraoperative data were compared for accessing pericholecystic adherence. RESULTS Sensitivity, specificity and accuracy of preoperative sonography for predicting the difficulty of LC was 79%, 98% and 96% respectively. CONCLUSION Preoperative sonography before LC might be a good predicting tool for pericholecystic adherence and difficulty of LC.
