摘要
[目的 ]研究端粒酶活性在卵巢恶性肿瘤组织发生、发展中的作用 ,探索端粒酶活化对卵巢恶性肿瘤的诊断、鉴别诊断和筛查的临床意义。 [方法 ]应用以 TRAP为基础的 PCR- EL ISA方法检测 2 3例卵巢恶性肿瘤及对应的癌旁组织、10例的交界性卵巢肿瘤组织、12例正常卵巢与 2 8例良性卵巢肿瘤组织的端粒酶活性。 [结果 ]在 2 3例卵巢恶性肿瘤中有 2 1例 (91.3% )端粒酶活性阳性 ,癌旁组织中 4例 (17.3% )阳性 ,交界性卵巢恶性肿瘤中 2例 (2 0 % )阳性 ,正常卵巢中未见增高 ,2 8例良性卵巢肿瘤中 2例 (7.1% )弱阳性。端粒酶在卵巢恶性肿瘤组中表达明显高于正常卵巢和良性卵巢组 (P<0 .0 5 )、交界性卵巢肿瘤组、癌旁组织组 (P值均 <0 .0 5 ) ;交界性卵巢肿瘤组高于正常卵巢和良性卵巢肿瘤组(P<0 .0 5 ) ,而在正常卵巢和良性卵巢肿瘤中表达无差异 (P>0 .0 5 )。各种病理类型的卵巢恶性肿瘤组织中端粒酶活化无差异 ,端粒酶活性增高在肿瘤的临床分期、恶性 (分化 )程度上差别显著 ,临床分期高、恶性度高的卵巢肿瘤其端粒酶活性也高。 [结论 ]端粒酶在卵巢恶变的过程中起重要作用 ,是卵巢恶性肿瘤形成中的普遍现象 ,端粒酶活性的检测有助于卵巢肿瘤的诊断和鉴别诊断 ,而且有助于预后的预测。
To evaluate the role of telomerase activity in the genesis and development of ovarian malignant tumors and its clinical significance The telomerase activity in 23 malignant tumor and their neighbor tissues,10 borderline or low malignant potential (LMP) tissues, 12 normal ovarian and 28 benign ovarian tumor tissues were detected by PCR ELISA based on TRAP (teromeric repeated sequences amplification protocol) The telomerase positive rate was 91 3%(21/23) in malignant tumor tissues, 17 3%(4/23) in neighbor tissues of malignant tumor, 20%(2/10) in LMP tissues, 7 1%(2/28) in benign tissues And it was negative in all of normal ovarian The level of telomease activity was significantly higher in LMP tissues than in benign ovarian tumor and normal ovarian(P<0 05) It was not significantly different between benign ovarian tumor and normal ovarian tissues (P>0 05) And it was not significantly different among different pathologic patterns either The telomerase activity had positive correlation with the clinical stage and differentiation of tumors [Conclusion] The expression of telomerase may play a major role in ovarian malignant tumors The detection of telomerase activity is useful for diagnosis and differential diagnosis of ovarian tumors, and predicting prognosis of patients with malignant ovarian tumors
