摘要
目的 比较射频消融术 (RFCA)前后血清心肌肌钙蛋白I(cTnI)及肌酸激酶同工酶 (CK MB)含量变化 ,对RFCA后微小心肌损伤 (MMD)的诊断价值。方法 对 35例行RFCA的室上性心动过速 (SVT)患者 ,排除其他器质性心脏病 ,进行上述 2项指标连续动态监测。 2 0例体检正常人做对照组。结果 RFCA前 35例患者cTnI、CK MB均在正常范围。cTnI在RFCA后即刻升高 ,由RFCA前 (0 11±0 0 2 ) μg/L升至 (0 5 8± 0 2 5 ) μg/L ,4h达峰值 (1 2 5±0 5 6 ) μg/L ,48h恢复到RFCA前水平。RFCA后即刻cTnI浓度平均升高达 (5 0± 2 9)倍 ,高于CK MB升高倍数 (P <0 0 1) ,cTnI达峰值时较RFCA前升高 (10 8± 4 5 )倍 ,明显高于CK MB的升高倍数 (P <0 0 1)。RFCA后cTnI升高超过界值占 94 2 %(33/35例 ) ,高于CK MB升高超过界值的比例 (P <0 0 1)。结论 RFCA可造成局部心肌坏死 ,通过连续动态监测血清cTnI含量变化得到证实。cTnI达峰早、即刻及峰值升高倍数明显高于CK MB ,是检测RFCA导致MMD的可靠指标。
Objective To compare the value of cTnI and CK-MB in diagnosing minor myocardial damage (MMD) before and after radiofrequency catheter ablation (RFCA). Methods 35 patients (16 men and 19 women) of supraventricular tachyarrhythmia underwent RFCA for treating tachycardias. cTnI and CK-MB were monitored before and after the procedure. Results Before RFCA, cTnI and CK-MB were at normal levels. But immediately after RFCA, cTnI obviously increased from (0.11±0.02) μg/L to (0.58±0.25) μg/L and to the peak concentration(1.25±0.56) μg/L at 4 h, then went down to the normal level whinth 48 h. The concentration of cTnI rose up to (5.0±2.9) times right after RFCA, which was significantly higher than that of CK-MB (P<0.01). Peak cTnI was (10.8±4.5) times as high as that of preoperation, which was significantly higher than that of peak CK-MB (P<0.01). After RFCA the percentage of cTnI rising above the cut-off was 94.2%, which was significantly higher than that of CK-MB (P<0.01). Conclusion RFCA may cause minor myocardial injury which can be reflected by ambulatory detecting cTnI level. cTnI is the best marker to measure the MMD caused by RFCA.
