犬控制性低血压下平均动脉压、脑局部血流与脉搏容积波幅的关系 被引量:1

摘要 目的 探讨犬控制性低血压时平均动脉压 (MAP)和脑表面局部血流 (rCBF)与脉搏容积波幅的关系。方法 犬全麻下经颅骨中缝旁开窗 ,激光多普勒测定脑皮质rCBF ,记录脉搏容积波幅值 (Pleth)。硝普钠降MAP为 6 0、5 0、4 0和 35mmHg。 结果 控制性降压在 80~ 35mmHg范围内rCBF几乎无变化 ;MAP与Pleth指数相关系数为 - 0 .93(P <0 .0 0 0 5 ) ,Pleth在 4 7mmHg时增加明显 (P <0 .0 5 ) ,其后Pleth随血压下降开始下降。 结论 犬MAP降至 35mmHg时 ,rCBF无明显变化 ;MAP降至 4 6mmHg后 ,Pleth随血压的降低从增加开始下降 。 Objective To observe the relationship of MAP and regional cereral blood flow (rCBF) to pulse volume amplitude (PVA) under controlled hypotension.Methods Mongrel dogs were intubated and mechanically ventilated with inhalation of isoflurane N 2O O 2 and continuous infusion of fentanyl and vecuronium.Cranial window was opened to fix the Laser Doppler flowmetry(LDF) probe,and a clip on Datex photoelectric plethysmography probe was placed at the lingua.Hypotension was achieved through nitroprusside infusion at MAP 60,50,40 and 35mm Hg.Results No significant correlation was observed between rCBF index and PVA index at MAP 80~35mm Hg,neither did bwtween rCBF index and PVA index,while good correlation(r=-0.93, P <0.0005)was found between MAP and PVA index.An obvious uptrend ( P <0.05) was detected until MAP decreased from control(80mm Hg) to 46mm Hg.Conclusions Excellent correlation was found in MAP and PVA index at MAP 80~35mm Hg,while 46mm Hg is the turning point,but above control in the end.The rCBF in canine was intact even when MAP falls to 35mm Hg in our study.
