摘要 目的 探讨钬激光治疗食管支架置入术后再狭窄的疗效。方法 应用钬激光治疗 9例食管支架置入术后再狭窄的患者。对向腔内生长的组织用 15W输出功率集中能量照射 ,必要时结合探条扩张改善视野 ,退镜时逆行照射 ;对穿过支架网眼生长的颗粒样组织用 <10W输出功率照射。 1例恶性狭窄激光照射后接受光动力疗法 (PDT)治疗。结果 治疗前基本完全堵塞的狭窄段 ,治疗后均得到再通 ,内镜可以顺利通过 ,吞咽困难明显改善 ,无并发症发生。结论 钬激光可以安全有效地再通食管支架置入术后的再狭窄 。 Objective To investigate the efficacy of Ho∶YAG laser palliation of recurrent esophageal obstruction after metal stents insertion. Methods Nine recurrent obstruction patients who had underwent metal stents insertion were treated with Ho∶YAG laser in combination with dilation. One patient was added a photodynamic therapy(PDT). Results Esophagus of 9 patients almost entirely obstructed before laser treatment, endoscope can pass beyond the lesion freely after treatment. Excellent palliation of the dysphagia was seen in all of the patients. There were no major complications. Conclusions Ho∶YAG laser therapy has remarkable efficacy on recurrent esophageal obstruction after metal stents insertion, combined with dilation and/or PDT which could enhance efficacy significantly.
