摘要
目的 :探讨肝转移瘤的CT诊断价值。材料与方法 :回顾性分析 10 0例肝转移瘤病灶形态、大小、密度、数目、特殊征象及其肝内分布 ,病灶增强前后大小比较和肝外转移等CT表现。结果 :6 5例转移灶 3个以上 ,10例病灶 2个 ,2 5例病灶仅有 1个 ;71例病灶位于右肝或主要位于右肝 ,11例左、右叶病灶相等 ,18例病灶位于左叶或主要位于左叶 ;97例肝转移瘤病灶均呈低密度 ,3例合并脂肪肝者 2例病灶呈高密度 ,1例呈等密度 ;2 0 6个转移灶中 12 5个平扫病灶较增强后为小 ,占6 0 7% ,81个病灶大小增强前后基本相同 ,占 39 3% ;牛眼征、囊性坏死征、液—液平征和肿瘤内钙化发生率分别为 8%、3%、 2 %和 4 % ;18例CT检查的同时发现肝外转移。结论 :病灶大小不等、多发弥漫和中心性坏死 ,边缘强化是肝转移瘤的基本CT表现 。
Purpose:To discuss the diagnostic evaluation of CT findings in hepatic metastases.Materials and Methods:100 Cases were retrospectively analyzed including the morphology,density,number,specific features,size of pre and post enhancement,intrahepatic distribution and extrahepatic metastatic lesions by CT scanning.Results:More than 3 lesions were showed in each of 65 cases,2 for each of 10 cases,and only 1 in 25 cases.71 cases revealed lesions in right lobe of liver or majority so,11 cases showed the equality of distribution in left and right lobes and 18 cases with lesions in or majority in left lobe.3 cases with fatty liver showed lesions in high density for 2 and another showed isodensity,lesions of the rest all were showed low density.Among 206 metastatic lesions,125 showed their sizes smaller than the post enhanced ones (60 7%) and the other 81 seemed to be in equal size before and after enhancement (39 3%).The rates of occurrence for bull eye sign,cystic necrosis sign,fluid-fluid level sign and intratumoral calcification were 8%,3%,2%and 4% respectively.Extrahepatic metastases occurred in 18 cases during CT examination.Conclusion:The variable size of the lesions,diffuse multiplicity and low density central necrosis with marginal enhancement were the basic CT manifestations.CT contrast scanning should be crucial to evaluate the exact sign of the metastatic lesion.
