烧伤合并内毒素血症骨髓粒单系造血反应 被引量:1

摘要 目的 探讨烧伤合并内毒素血症骨髓粒单系造血反应及骨髓细胞病理改变。方法 采用细胞计数和细胞培养观测总体表面积 (TBSA) 2 0 %Ⅲ度烧伤合并内毒素 (1μg/g体重 )腹腔注射后 1周内小鼠的外周血粒细胞和单核细胞总数及骨髓粒 -巨噬细胞集落形成单位 (CFU GM)动态变化 ,并用光、电镜和骨髓细胞原位凋亡检测 (TUNEL)法观察骨髓细胞病理改变和细胞凋亡。结果 烧伤合并内毒素注射后 6小时内 ,外周血粒细胞和单核细胞总数、骨髓CFU GM略增多 ,以后逐渐减少 ,1~ 5天外周血粒细胞和单核细胞总数、以及 12小时~ 5天的骨髓CFU GM显著低于正常对照组。烧伤注射后早期 ,可见较多的凋亡阳性细胞及骨髓细胞核固缩、碎裂、凋亡小体、线粒体肿胀、空泡化等病理改变。结论 烧伤合并内毒素血症早期骨髓粒单系略呈增生反应 ,继而呈造血抑制 ,与单烧组、单注内毒素组比较 ,具有抑制出现时间早、程度重、持续时间长的特点。 Objective To investigate the response of bone marrow granulopoiesis and monocytopoiesis and the pathological change of bone marrow after burns combined with endotoxemia in mice.?Methods The dynamic change of circulating granulocyte and monocyte, CFU GM were determined with cell count and cell culture respectively in 7 days after 20% TBSA full thickness skin burns combined with endotoxin (LPS, 1μg/g body weight) injection. The pathological change and apoptosis in bone marrow were detected by LM, EM and TUNEL.?Results The total amount of circulating granulocytes and monocytes,CFU GM in bone marrow increased slightly within 6h after burns combined with LPS injection,then decreased gradually. The total amount of circulating granulocytes and monocytes at the period from 1st to 5th day and,at CFU GM 12th hour to 5th day decreased respectively.Extensive apoptotic bone marrow cells and a series of pathological changes including pyknosis,karyorrhexis, apoptotic body, mitochondrion swelling, vacuolation, et al,were found in early stage after burns combined with endotoxemia.?Conclusion Bone marrow granulopoiesis and monocytopoiesis are enhanced at early stage of burns combined with endotoxemia, then suppressed. The suppression is more severe, much earlier and lasted longer compared with simple burns or simple LPS injection groups.
