感觉神经植入皮瓣重建感觉功能的实验研究与临床应用

摘要 为了重建无感觉皮肤的感觉功能,对感觉神经植入皮瓣重建感觉功能进行了实验研究,在大白鼠下腹部两侧制成带腹壁浅血管神经的岛状皮瓣,将与腹壁浅血管伴行的皮神经在进入皮下组织处切断,再植入到皮瓣中心真皮下层为实验组,将上述皮神经自股神经处切除为对照组。利用HE染色、Gless氏神经轴突镀银染色及胆碱酯酶组织化学染色方法观察,术后2月时植入神经周围出现再生,术后3月植入神经再生明显,术后4月植入神经基本建立了和正常相似的神经末稍。根据上述实验结果,本组临床25例将知名感觉神经植入到皮瓣用来重建手足失感觉的皮肤感觉功能。随访1~2年,痛、温、触觉恢复正常,两点鉴别觉在手部11~20mm,足部35~40mm,有3例在植入感觉神经近端用2%奴夫卡因封闭后皮瓣感觉消失,说明皮瓣的感觉已由植入的感觉神经支配。 Implant sensory nerve into the flaps to recover sentience have experimental studied. Because transplantation of the flaps often defect suitable butt joint nerve. Surrounding of implanting sensory nerve has regeneration after operation 2 months and it's obvious regeneration after operation 3 months and it's similar normal nerve ending after operation 4 months. In accordance with experimental result, implanted sensory nerve into flaps to recover sentience in the hands and feet on 25 cases. After 1-2 years follow-up, the pain, temperature and tuctile are recovered to normal and two points discrimination is 11-20 mm in the hands and 35-40 mm in the feet. The anesthesia of the flaps after the implanting sensory nerve at proximal are blocked by 2% procarine on 3 cases which show that sentience of the flaps are controuled by implanting sensory nerve.
