摘要
目的 分析在出血后不同时间段内行纤维胃镜检查的各组镜检阳性率 ;探讨急性上消化道出血的病因 ,以及与年龄的关系。方法 回顾分析门诊及住院经纤维胃镜检查 2 0 0 7例患者中检出的 95例上消化道出血的临床资料。结果 95例中 ,青壮年组有 66例 ,老年组有 2 9例。 3 2例二十指肠球部溃疡出血患者中 ,青壮年组有2 8例 ,老年组有 4例 ,阳性率分别为 4 2 4 2 %和 13 79% ,两组差异比较有高度显著性 11例胃癌出血患者中 ,青壮年组有 4例 ,老年组有 7例 ,阳性率分别为 6 0 6%和 2 4 14 % ,两组比较差异有高度显著性。结论 对急性上消化道出血患者越早行胃镜检查 ,镜检阳性率越高。消化性溃疡居上消化道出血各种病因之首位。青壮年组十二指肠球部溃疡出血患者明显多于老年组 ,老年组胃癌出血患者明显多于青壮年组。
Objective To analyze the positive rate of cases examined by gastroscope in different time after bleeding.To investigate the causes of acute upper digestive tract bleeding and the different causes in different age groups.Methods We looked back and analyzed the 95 cases with acute upper digestive tract bleeding in the 2007 cases examined with fiber gastroscope who were outpatients or inpatients in the hospital.Results Among the 95cases,66 cases were in the young group and 29 in the old.Among32 bleeding cases with duodenal bulbar ulcer,28 cases were in the young group and 4 in old.The positive rate was 42.42%and 13.79%,respectively.There was significant difference between the young and old groups ( P <0 01).Among ll bleeding cases with gastric cancer,there were 4 cases in young group and 7 in old.The positive rate was 6.06% and 24 14%,respectively.There was significant difference between the two groups (P <0 01).Conclusion The more early we examined the patients with acute upper digestive tract bleeding by gastroscope,the higher the positive rate was.The patients with duodenal bulbar ulcer bleeding in young group was evidently more than in old group.The patients with gastric cancer bleeding in old group was evidently more than in young group.
