摘要
目的评价关节内骨折手术治疗后及时补充外源性玻璃酸钠(sodiumhyaluronate,SH)的临床疗效。方法118例关节内骨折病例随机分为实验组(A)和对照组(B),每组59例。手术复位、内固定后,A组关节腔内注入1%SH注射液2ml,1次/周,共5次,B组术后不进行SH注射。于术后3d、7d、15d、1个月、3个月、6个月分别对A、B两组的疼痛、肿胀、关节活动度、骨折愈合、关节内粘连等指标进行观察分析。结果术后7d内,关节疼痛、肿胀两组之间无明显差异,术后15d～3个月两组之间存在着明显差异,A组症状缓解优于B组(P<0.05);术后1个月内,关节活动度两组之间无明显差异,1个月后A组活动度优于B组(P<0.05)。术后3个月内,骨折愈合两组无明显差异,6个月后A组较B组愈合快。6个月后再次手术发现,A组关节内粘连的程度及性质较B组轻微、疏松。结论术后关节腔内注入SH,对术后患者的关节疼痛、肿胀具有一定的缓解作用,同时能增加关节活动度,促进骨折愈合,减轻关节内粘连,是预防关节内骨折手术治疗并发症的有效辅助治疗方法。
Objective To evaluate the clinical effects of extraneous sodium hyaluronate injection (SH) into the articular space after joint fracture surgery. Method 118 patients were randomly and averagely divided into experimental group (group A) and control group (group B). After operative restoration and internal immobilization, 1% SH injection (2 ml) was injected intra articularly into patients of group A immediately and another 4 times at a weekly intervals. Patients of group B were not given SH injection after operation. Clinical signs including articular pain, joint edema, range of movement (ROM), healing of bone fracture were monitored on the 3 d,7 d,15 d, and at the end of 1st month , the 3rd month, the 6th month after surgery. Result During the 7d after operation, there were no significant differences for joint pain and edema between the two groups. During the 15 d ～ 3rd month,there were significant differences between two groups(P< 0.05). The significant differences in range of movement were noted after 1 month. There was no significant difference in the healing of bone fracture during 3 months after operation, but after 6 months, the patients in group A showed faster healing than the patients in group B. The patients in group A showed less and milder articular adhesion than the patients in group B owning to SH injection. Conclusion Extraneous SH injection into the fracture joint can alleviate joint pain and edema, improve the ROM of joint, stimulate the bone healing and prevent articular adhesion. Intra articular injection of SH is an effective supplementary way to prevent the postoperative complications of articular fracture.
