摘要
目的 探讨小儿肺炎时血清心肌酶活性改变及其临床意义。方法 测定肺炎组 2 4例患儿及对照组 18例体检正常儿的血清心肌酶活性 ,并进行两组间的比较。结果 肺炎组患儿的血清乳酸脱氢酶 (LDH)、α-羟丁酸脱氢酶 (α-HBDH)、磷酸肌酸激酶 (CPK)的活性与对照组的比较 ,差别有显著性意义 (P <0 0 5 )。轻症肺炎患儿仅有LDH或LDH和α -HBDH或LDH和CPK活性轻度增加。重症肺炎患儿LDH和α-HBDH或LDH和CPK或其中的三项活性明显增加 ,临床可表现为心力衰竭、心肌病变、中毒性肠麻痹等。结论 肺炎时常有一项或多项心肌酶活性呈不同程度增高 ,以LDH活性增高最常见 ,重症肺炎时常有二项或三项心肌酶活性同时增高 ,因此临床上应进行心肌酶活性测定 ,以便早期发现心肌损害 ,及时对心肌采取保护性治疗。
Objective To determine the changes of dehydrogenase(LDH),creatine phosphokinase(CPK) and α-hydroxybutyric dehydrogenase(α-HBDH),and explore its clinical significance in the children with pneumonia.Methods The serum LDH,CPK and α-HBDH were measured in 24 children with pneumonia and 18 normal children.Results The serum LDH andα-HBDH of pneumonia group was significantly higher than that of control group(P<0.01),and the serum CPK of pneumonia group was higher than that of control group(P<0.05).Level of LDH or LDH and α-HBDH or LDH and CPK in the serum was increased in the mild pneumonia group,and level of LDH and α-HBDH or LDH and CPK or three items in the serum were obviously increased in the severe pneumonia group.There showed heart failure,cardiac muscle pathological changes,toxical paralytic ileus etc.in clinical findings.Conclusion One of three items increases differently in children with pneumonia at least,usually,the serum LDH increases.When two or three of items increase at the same time in the sever pneumonia,myocardial damage occurs.Therefore,the determination of serum myocardium enzyme active should be emphasized in the children with pneumonia.
