摘要
目的 确定胫骨内侧嵴的角度 ,设计一种角度适合于胫骨内侧固定的钢板。方法 取尸体胫骨 40根 ,分别测量其中的5点 ,根据统计结果 ,用 31 7L型钢材 ,预制成内角为 90°的V形钢板 ,用公式计算抗弯强度 ,并实测抗扭强度 ,然后在临床应用 59例 63处骨折 ,钢板全部固定于胫骨内侧嵴。结果 靠胫骨近、远端a、e点的平均角度为 90°± 5° (85°~ 95°) ,中间b、c、d三点的角度为 90°± 3° (87°~ 93°)。V形钢板比直型钢板的抗弯强度大 ,抗扭强度相似。 45例平均随访 1 3个月 ,骨折均愈合 ,未发生钢板断裂或弯曲。结论 胫骨内侧嵴是重要的张力侧 ,按照力学分析 ,钢板应固定于胫骨的内侧嵴 。
Objective To confirm the angle of tibia inner crista and design a plate fit for fixation of the inner tibia.Methods Forty tibiae of corpse were taken and five points among the tibia were separately measured.The plate was made with 317L steels and its internal angle was 90° as a 'V' shape.The plate internal angle was alike as tibia inner angle according to the measured results.It was used for 59 patient with 63 tibia fracture.The plate was fully fixed on the inner crista of tibia.Results Measured the two point on tibia proximal and distal end,their angle was 90°±5°(85°~95°),the angle of three point on middle part of tibia was 90°±3°(87°~93°).The intensity of antibend of V shaped plate was larger than straght plate and the intensity of anti-torsion was similar between straight plate and V shaped plate. The effect was satisfactory on the clinical application.The bone nonunion and plate bend were not happened in this series.Conclusion The tibia inner crista is the most important tensility position on the tibia-fibula fracture.According to the mechanics analysis, the plate should be fixed on inner crista of tibia.The plate may be fixed on tibial inner crista and can be used to treat tibia-fibula fracture.
