基于寸口脉搏波血流动力学分析的体外循环灌注效果评价 被引量:2

Evaluation on perfusion effects of cardiopulmonary bypass based on hemodynamics analysis of wrist pulse wave

摘要 目的:通过观察体外循环不同灌注模式下寸口脉搏波频谱的变化规律,建立基于寸口脉搏波监测的体外循环搏动灌注效果评价方法。方法:纳入60例行体外循环术治疗的心脏病患者,随机分为搏动灌注(pulsatile perfusion,PP)组和非搏动灌注(nonpulsatile perfusion,NP)组,每组各30例。应用Power Lab数据采集分析系统记录脉搏波指标,比较两组研究对象在体外循环前、体外循环中的脉搏波频域指标和血流动力学指标变化情况。结果:PP组研究对象的能量等值压(energy equivalent pressure,EEP)、富余血流动力学能量(surplus hemodynamic energy,SHE)、血液动力学总能量(total hemodynamic energy,THE)等血流动力学指标显著高于NP组(P<0.05),PP组脉搏波能量高于NP组(P<0.05)。结论:脉搏波频域指标可用于监测体外循环的血流动力学指标,并能作为评价有效性搏动灌注的客观指标。 Objective: To observe the changes of wrist pulse wave spectrum in different perfusion patterns of cardiopulmonary bypass,and establish the evaluation methods for pulsatile perfusion of cardiopulmonary bypass based on monitoring wrist pulse wave. Methods:Sixty patients with cardiac diseases who were treated by cardiopulmonary bypass were included and randomly divided into the pulsatile perfusion( PP) group and the non-pulsatile perfusion( NP) group,30 cases in each group. The wrist pulse wave indexes were recorded by Power Lab physiological system. Before and in the course of cardiopulmonary bypass,the changes in frequency domain indexes of pulse wave and hemodynamic indexes were compared between the two groups. Results: The hemodynamic indexes including energy equivalent pressure( EEP),surplus hemodynamic energy( SHE) and total hemodynamic energy( THE) in the PP group were significantly higher than those in the NP group( P<0.05),and the pulse wave energy in the PP group was higher than that in the NP group( P<0.05).Conclusion: Frequency domain indexes of pulse wave can monitor the hemodynamic indexes of cardiopulmonary bypass,and be used as an objective indicator to evaluate the effects of pulsatile perfusion.
