摘要
为探讨耻骨直肠肌部分保留联合括约肌间开创引流治疗高位复杂性肛瘘的临床效果,将2016年9月至2018年1月于我科就诊的86例高位复杂性肛瘘患者随机分为对照组和观察组,各43例,对照组行肛瘘低位切开高位挂线术治疗,观察组行耻骨直肠肌部分保留联合括约肌间开创引流术治疗,对比2组总体疗效、创面愈合时间,以及术后疼痛程度(VAS评分)、创面分泌物及肛门括约肌功能。结果显示,2组治愈率比较差异无统计学意义,P>0.05;观察组创面愈合时间短于对照组,术后第3天、第7天VAS评分和创面分泌物情况评分,以及术后2个月的肛门功能评分均显著低于对照组,P <0.05。结果表明,与肛瘘低位切开高位挂线术相比,耻骨直肠肌部分保留联合括约肌开创引流术治疗高位复杂性肛瘘,可在保证临床疗效的情况下,有效减轻患者疼痛程度,减少创面分泌物,促进肛门括约肌功能恢复,缩短创面愈合时间,值得临床推广应用。
To investigate the clinical effect of a combined procedure(puborectal muscle-partly saving and intersphincteric wound-opening drainage)in the treatment of high complex fistula in ano,80 patients with this fistula who were admitted and received surgery in author's dept.(2016-09-2018-01)were randomly divided into control group(43 cases,adopted the procedure of low incision & high thread-drawing for fistula)and observation (43 cases,adopted above combined procedure),then their following indexes such as whole effect,wound-surface healing time,VAS score on postoperative pain,wound-surface secretion and sphincter function were compared between the two groups.As results,both groups' curative rate had no statistical difference(P>0.05);but in wound-surface healing time,in the VAS score on the 3rd,17th day after surgery and the score on wound-surface secretion,and in the score on anal function 2 months after surgery observation group was significantly shorter,lower and lower than control group(all,P<0.05).Resuhs show that compared with the procedure of low incision & high thread-drawing for fistula,the procedure applied in observation group can not only ensure clinical effect,but effectively alleviate patients' pain degree,reduce wound-surface secretion,promote anal sphincter function recovered,shorten wound-surface healing time,in the treatment of high complex fistula in ano.It is worthy of clinical popularization and application.
