摘要
目的分析佩戴颈托联合经皮激光椎间盘减压术(PLDD)治疗颈性眩晕症的临床效果。方法选择2013-01-2015-06我院收治的100例颈性眩晕症患者作为研究对象,按随机数字表法分为对照组与观察组每组50例,两组均采用PLDD术治疗,观察组联合佩戴颈托干预,采用改良Macnab标准评定两组治疗效果,应用数字评分法(NRS)评定患者症状及体征的改善情况,治疗前后均进行X线检查,测定患者颈椎过伸过屈位角位移(AD)及水平位移(HD)情况,统计两组并发症发生率。结果①术后2年观察组总优良率高于对照组,比较差异有统计学意义(P<0.05)。②术后2年,两组眩晕、视物不清、恶心、头痛、颈部疼痛等症状NRS评分均降低,观察组各症状评分均低于对照组,比较差异有统计学意义(P<0.05)。③两组术前、术后2年AD、HD均无明显变化,比较差异无统计学意义(P>0.05),且两组并发症发生率比较差异无统计学意义(P>0.05)。结论佩戴颈托联合PLDD治疗颈性眩晕症可改善患者眩晕及伴随症状,促进颈椎功能恢复,提高颈椎稳定性。
Objective To analyze the clinical effect of wearing cervical collar combined with percutaneous laser disc decompression (PLDD)in the treatment of cervical vertigo.Methods From January 2013 to June 2015,100 patients with cervical vertigo treated in our hospital were selected and randomly divided into the control group and the observation group,with 50 cases in each group.The two groups were treated by PLDD,and the observation group was additionally given intervention with cervical collar. The curative effect and the improvement of symptoms and signs were evaluated with modified Macnab and numerical rating scale (NRS) respectively.The X-ray examination was performed before and after treatment to determine cervical over flexion-extension angle displacement (AD)and horizontal displacement (HD),and the incidence of complications was statistically analyzed.Results ①The total excellent and good rate of the observation group was higher than that of the control group at 2 years after operation (P<0.05).②At 2 years after operation,NRS scores of vertigo,unclear vision,nausea,headache and neck pain were all decreased,and the symptom scores in the observation group were lower than those in the control group (P<0.05).③There were no significant changes in AD and HD in the two groups before operation and at 2 years after operation (P>0.05),and there was no significant difference in the incidence rate of complications between the two groups (P>0.05).Conclusion Wearing cervical collar combined with PLDD can relieve vertigo and accompanying symptoms in patients with cervical vertigo,promote the recovery of cervical vertebra function and improve the stability of cervical vertebra.
