不同构型3枚螺钉固定稳定型股骨颈骨折的生物力学比较研究 被引量:4

摘要 目的对不同构型3枚空心加压螺钉固定稳定型股骨颈骨折进行生物力学及有限元分析比较,为临床治疗稳定性股骨颈骨折提供理论依据。方法用倒、正等腰三角形及反V构型的3枚空心加压螺钉固定稳定型股骨颈骨折的人工股骨标本,并采用生物力学机测试、比较3种空间构型的抗压、抗扭转性能及最大垂直载荷。结果反V构型组和倒等腰三角形构型组的抗压及最大垂直载荷性能明显优于正等腰三角形构型组,差异均有统计学意义(P <0.05),但抗扭转性能差异均无统计学意义(P> 0.05)。反V构型组在抗压、抗扭转性能及最大垂直载荷方面,参数的均值高于倒等腰三角形构型组,但差异无统计学意义(P> 0.05)。结论治疗稳定型股骨颈骨折应优先选用反V构型的3枚空心加压螺钉,其生物力学效果可能更好。 Objective To conduct a comparison by biomechanical and finite element analysis among three different configurations of cannulated compression screws in fixation for stable femoral neck fracture and offer a theoretical foundation for treatment of femoral neck fracture. Methods The specimen fixed by three 6.5 mm cannulated screws with inverted isosceles triangle, upright isosceles triangle as well as reverse V configuration, was measured on the biomechanical machine. The compression strength, torsibility and the maximum vertical loading were compared among the three configurations of crews. Results The compression strength and maximum vertical loading reverse V shaped and the inverted isosceles triangle shaped screws showed much better than that of the upright isosceles triangle shaped ones with statistical differences(P < 0.05), but there were no significant differences in torsibility. The reverse V shaped screws showed better compression strength, torsibility and the maximum vertical loading, than the inverted isosceles triangle shaped screws, but the difference was not statistically significant between the two shapes. Conclusions The reverse V configuration of screws should be utilized in treating stable femoral head neck fracture.
