摘要
【目的】探讨益气活血方对急性颈髓损伤合并呼吸衰竭患者呼吸系统的影响。【方法】将53例急性颈髓损伤合并呼吸衰竭患者随机分为对照组(25例)和治疗组(28例),对照组给予西医常规治疗,治疗组在西医常规治疗基础上联合中药益气活血方治疗,疗程1周。比较2组患者治疗前后呼吸功能[氧合指数(PaO_2/FiO_2)、肺顺应性和死腔容量/潮气量(Vd/Vt)]、相关感染指标[降钙素原(PCT)、C反应蛋白(CRP)、白细胞(WBC)]和急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分的变化情况,并观察2组患者的不良反应情况。【结果】(1)2组患者治疗7 d后的Vd/Vt、肺顺应性、PaO_2/FiO_2及治疗组治疗3 d后的肺顺应性、PaO_2/FiO_2均较治疗前明显改善(P<0.05);与对照组同期比较,治疗组治疗3 d和7 d后的肺顺应性明显改善,PaO_2/FiO_2明显升高(P<0.05)。(2)治疗后,2组患者PCT、CPR、WBC等炎症指标均较治疗前有不同程度下降,其中对照组治疗7 d后的PCT、WBC及治疗组治疗3 d和7 d后的PCT、WBC下降较明显,差异有统计学意义(P<0.05);治疗组治疗7d后的APACHEⅡ评分明显下降(P<0.05),而对照组下降不明显(P>0.05);与对照组同期比较,治疗组治疗7 d后的PCT、WBC水平及APACHEⅡ评分均下降明显(P<0.05)。(3)在整个治疗过程中,2组患者均无出血、皮疹、瘙痒、胸闷等症状发生,肝肾功能均无异常改变,2组不良反应情况比较,差异均无统计学意义(P>0.05)。【结论】益气活血方可增强颈髓损伤合并呼吸衰竭患者的呼吸功能,减轻感染的程度,有效改善病情。
Objective To investigate the effect of Yiqi Huoxue Recipe(YHR;a Chinese herbal prescription with the actions of strengthening Qi and activating blood)on respiratory system in acute cervical spinal cord injury patients complicated with respiratory failure. Methods A total of 53 cases of acute cervical spinal cord injury patients complicated with respiratory failure were randomly divided into control group(25 cases)and treatment group(28 cases). Both groups were given conventional western medicine treatment,and additionally the treatment group was given oral use of YHR,the treatment lasting for one week. Before and after treatment,the respiratory function[the ratio of arterial oxygen partial pressure to fractional inspired oxygen (PaO2 / FiO2), pulmonary compliance and dead space-to-tidal volume ratio(Vd/Vt)], inflammation associated parameters [procalcitonin (PCT)content,C-reactive protein(CRP)level and white blood cell(WBC)count],and Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ)scores in the two groups were compared. Meanwhile, the adverse reaction was also monitored in the two groups. Results (1)After treatment for 7 days, Vd/Vt, pulmonary compliance and PaO2 / FiO2 ratio in the two groups were improved,and the pulmonary compliance and PaO2/FiO2 ratio in the treatment group after treatment for 3 days were also improved, the difference being significant compared with those before treatment(P<0.05). Compared with those in the control group at the same period,the pulmonary compliance was much improved and PaO2/FiO2 ratio was markedly increased in the treatment group after treatment for 3 and 7 days(P<0.05).(2)After treatment,the inflammation associated parameters of PCT,CRP and WBC count in the two groups were decreased to various degrees,and the differences of PCT and WBC count count in the control group after treatment for 7 days as well as in the treatment group after treatment for 3 and 7 days were significant(P < 0.05 compared with those before treatment).
