摘要
目的探讨ICU转出患者家属的依恋心理、经验性回避与迁移应激之间的相关性。方法采用方便抽样法,2017年5—9月选取于西安交通大学第一附属医院ICU住院治疗转出的患者家属272名为研究对象,使用亲密关系体验问卷、ICU转出患者家属迁移应激量表和接纳与行动问卷-第2版调查其依恋心理、迁移应激和经验性回避,并使用Amos 17.0软件进行中介效应检验。结果本研究共发放问卷272份,回收有效问卷256份,有效问卷回收率为94.1%。患者家属的迁移应激总分为(59.84±7.53)分,依恋焦虑得分为(3.51±0.92)分,依恋回避得分为(3.12±0.87)分,经验性回避得分为(16.61±6.24)分。迁移应激、依恋回避与经验性回避之间具有相关性(P<0.05)。经验性回避在依恋回避与迁移应激之间起部分中介作用,且中介效应占总效应的41.4%。结论医务工作者应重视ICU转出患者家属的心理健康状况,引导和鼓励家属倾诉真实感受,并帮助家属采取积极的应对策略,以促进其身心健康,并为患者提供良好的社会支持。
Objective To explore the relationship between attachment, experiential avoidance and relocation stress in families of patients transferred from the Intensive Care Unit (ICU) .Methods Totally 272 families of patients transferred from the ICU of the First Affiliated Hospital of Xi'an Jiaotong University were selected by convenient sampling between May and September 2017. Their attachment, relocation stress and experiential avoidance were investigated with the Experiences in Close Relationships Questionnaire-Revised, Relocation Stress Scale for the Families of Patients Transferred From ICU and Acceptance and Action Questionnaire-2nd Edition. Amos 17.0 was used to test the mediating effects.Results Totally 272 questionnaires were distributed, and 256 valid questionnaires were collected, with a valid collection rate of 94.1%. The total relocation stress score was (59.84±7.53) ; the attachment anxiety score was (3.51±0.92) ; the attachment avoidance score was (3.12±0.87) ; and the experiential avoidance score was (16.61±6.24) . There was a correlation between relocation stress, attachment avoidance and experiential avoidance (P<0.05) . Experiential avoidance had mediating effect between attachment avoidance and relocation stress, and the mediating accounted for 41.4% of the total effect.Conclusions Medical workers shall pay attention to the psychological health of the families of patients transferred from ICU, encourage them to pour out their real feelings, help them take positive countermeasures, improve their physical and mental health, and provide sound social support for the patients.
