摘要
慢性气道炎症性疾病的重要病理生理和临床表现为气道黏液高分泌,临床上存在许多慢性气道炎症性疾病患者因气道黏液高分泌而出现疾病反复、病情加重。对本病的病机认识要重视"痰"的作用,治疗过程中也应积极化痰。目前,中医药已从细胞因子、炎症介质、黏蛋白、分子信号通路等方面对气道黏液高分泌影响进行探讨,干预形式多样化,有以单味化痰中药的作用研究为主,也有复方制剂、中成药制剂,甚至出现系列方剂组合;处方来源多元化,有传统辨证论治、经方或名老中医经验方化裁以及自拟处方等。然而,中医药对气道黏液高分泌的相关研究起步较迟,对很多药物的干预机制研究不够透彻,目前该类研究多集中在黏蛋白环节,且以动物实验居多,临床试验相对较少,缺乏大样本、多中心的试验研究,存在许多不足之处。因此,在今后的研究中,基于中医学相关理论的指导,应多从导致气道黏液高分泌的多条信号通路、基因层面等多角度出发进行研究,扩大临床试验,深入挖掘中医药干预气道黏液高分泌机制,以期能获得更多的思路与方法。
The important pathophysiological and clinical manifestations of chronic airway inflammatory diseases are airway mucus hypersecretion. Many patients with chronic airway inflammatory diseases suffer from repeated diseases and aggravated conditions due to airway mucus hypersecretion. We should pay attention to the role of phlegm in understanding the pathogenesis of this disease,and actively remove phlegm in the course of treatment. At present,TCM has explored the effects of cytokines,inflammatory mediators,mucin and molecular signaling pathways on airway mucus hypersecretion. Intervention forms are diversified. There are mainly single Chinese medicines for eliminating phlegm,and there are also compound preparations,Chinese patent medicine preparations,and even a series of prescription combinations. Prescription sources are diversified,including traditional syndrome differentiation and treatment,classical prescriptions or experienced prescriptions of famous and old Chinese medicine,and self-made prescriptions. However,the research on airway mucus hypersecretion in TCM is relatively late,and the intervention mechanism of many drugs is not thorough enough. At present,most of these studies focus on mucin,and most of them are animal experiments,clinical trials are relatively few,lack of large sample,multi-center experimental studies,and there are many shortcomings. Therefore,in the future research,based on the guidance of relevant theories of TCM,we should do more research from multiple signaling pathways and gene levels that lead to airway mucus hypersecretion,expand clinical trials,and dig deeply into the mechanism of intervention of TCM on airway mucus hypersecretion,so as to obtain more ideas and prescriptions.
