主动脉内球囊反搏联合体外膜肺氧合治疗急性心肌梗死合并心源性休克1例 被引量:7

IABP combined with ECMO for the treatment of acute myocardial infarction complicated with cardiogenic shock:one case report

摘要 1病例资料患者,男,70岁,以“突发胸痛1.5h”为主诉于2018-01-22急诊入院,入院体检:体温36.5℃,脉搏150次/min,呼吸20次/min,血压64/45mmHg(1mmHg-0.133kP.a)。神志清楚;无颈静脉怒张;双肺呼吸音清,未闻及干湿性啰音;心率165次/min,心律不齐,心音较低钝,第1心音强弱不等,各瓣膜听诊区未闻及杂音。既往有葡萄糖-6-磷酸脱氢酶缺乏症、肛瘘手术后3年病史,否认“高血压、糖尿病”病史,吸烟40年,20支/d,饮酒40年,50g/d。 The incidence rate of cardiogenic shock (CS) in patients with ST-elevation myocardial infarction (STEMI) is about 8%.Cardiogenic shock is the leading cause of in-hospital mortality with STEMI,especially in elderly patients.Early revascularization can improve the survival rate of the patients,but the overall prognosis is still poor.Mechanical circulatory support (MCS) may improve outcomes for the patients.We present a case that successfully combined intra-aortic balloon pump(IABP)with extracorporeal membrane oxygenation (ECMO) in the treatment of STEMI with CS.
