足部空间姿态评估分析系统在ACL重建前后步态分析中的应用 被引量:6

Analysis of gait before and after anterior cruciate ligament reconstruction using foot space pose assessment system

摘要 目的用足部空间姿态评估分析系统(Sennotech Insole X 1.0)测量关节镜下前交叉韧带(ACL)重建手术前后患肢步态参数,并与正常人步态参数进行比较,评估ACL重建患者步态变化特点。方法纳入自2017-01—2017-08诊治的28例ACL损伤,均在关节镜下用自体半腱肌、股薄肌重建ACL,采用Sennotech Insole X 1.0系统对术前及术后3个月足底压力时相变化及参数进行测定(观察组)。纳入同期28名健康成人作为对照组并采集相应参数。结果观察组术后3个月患肢步幅、步高、着地冲击力、支撑相、着地期、平足期、着地仰角、离地仰角、最大摆动速度较术前明显增加,而蹬伸期、摆动相较术前明显降低,差异有统计学意义(P <0.05)。观察组术后3个月患肢步幅、步高、着地冲击力、支撑相、着地期、平足期、着地仰角、离地仰角、最大摆动速度、蹬伸期、摆动相与正常人差异无统计学意义(P>0.05)。结论足部空间姿态评估分析系统可以量化评价ACL重建手术前后患肢步态变化特点,进而制定更加合理、有效、针对性强的治疗计划,以帮助患者更好地恢复膝关节生理运动功能。 Objective To measure the gait parameters of the affected limbs before and after arthroscopic anterior cruciate ligament (ACL) reconstruction with the assessment system of foot spatial attitudes (Sennotech Insole X 1.0) and compare with the gait parameters of normal people to evaluate the gait changes of patients with ACL reconstruction.Methods Twenty-eight cases of ACL injury treated from January 2017 to August 2017 were included.All patients underwent arthroscopic reconstruction of ACL with autologous semitendinosus and gracilis.The phase change and parameters of plantar pressure before and 3 months after operation were measured by Sennotech Insole X 1.0 system (observation group).Twenty-eight healthy adults served as control group and the corresponding parameters were collected.Results In the observation group,the limb stride,step height,ground impact forc'e,supporting phase,landing period,flat foot period,landing elevation angle,elevation angle and maximum swing speed were significantly increased.The extension period and swing period were significantly reduced after operation.The difference was statistically significant (P <0.05).In the observation group,there was no statistically significant difference in limb stride,step height,ground impact force,supporting phase,landing period,flat foot period,landing elevation angle,elevation angle and maximum swing speed 3 months after surgery compared with the control group (P >0.05).Conclusion The assessment system of foot spatial attitudes can quantitatively evaluate the gait changes of affected limb before and after ACL reconstruction,and then develop a more reasonable,effective and targeted treatment plan to help patients better restore their physiological movement function of the knee joint.
