摘要
目的探讨全程无缝隙护理方案对维持性血液透析合并肿瘤患者医院感染及遵医行为依从性的影响。方法选取2015年10月至2017年10月间湖北省黄冈市英山县人民医院收治的82例维持性血液透析合并肿瘤患者进行回顾性分析,根据护理方法不同分为无缝隙护理组和对照护理组,每组41例。无缝隙护理组在常规护理的基础上采用从医院到家庭的无缝隙护理方案护理,对照护理组采用常规护理措施,护理8周后,比较两组患者医院感染情况、遵医行为依从性及舒适度。结果无缝隙护理组患者医院感染发生率为7. 3%,显著低于对照护理组患者的24. 4%,差异有统计学意义(P <0. 05)。无缝隙护理组的饮食依从性、运动依从性、用药依从性、自我管理依从性及心理调节依从性均高于对照护理组患者,差异均有统计学意义(均P <0. 05)。两组患者护理干预前的舒适度评分比较,差异无统计学意义(P> 0. 05),护理干预后,两组患者舒适度评分均上升,且无缝隙护理组患者舒适度评分高于对照护理组患者,差异均有统计学意义(均P <0. 05)。结论采用全程无缝隙护理对维持性血液透析合并肿瘤患者进行护理,可有效降低患者医院感染发生率,提高遵医行为依从性,获得舒适体验,促进生活质量的提高。
Objective To explore the effect of complete seamless nursing on nosocomial infection and medical compliance in patients with tumors receiving maintenance hemodialysis. Methods A total of82 patients with tomors receiving maintenance hemodialysis tumors who admitted to Yingshan County People's Hospital from October 2015 to October 2017 were selected as the subjects and retrospectively analyzed. They were randomly divided into a seamless nursing group and a control nursing group with 41 patients in each group according to random digital table method. Patients in the seamless nursing group adopted the seamless nursing program from hospital to family on the basis of routine nursing. Patients in the control nursing group were given routine nursing measures. At 8 weeks after nursing,the nosocomial infection,compliance and comfort were compared between the two groups. Results The incidence of nosocomial infection was 7. 3% in the seamless nursing group which was significantly higher than 24. 4% in the control nursing group( P<0. 05). Dietary compliance,exercise compliance,medication compliance,self-management compliance and psychological regulation compliance were significantly higher in the seamless nursing group than in the control nursing group( all P<0. 05). There was no significant difference in the scores of comfort between the two groups before nursing intervention( P>0. 05). After nursing intervention,the scores of comfort degree increased significantly in the two groups( P<0. 05),and the scores of comfort in the seamless nursing group were significantly higher than in the control nursing group( P<0. 05).Conclusion Complete seamless nursing care can effectively reduce the incidence of nosocomial infection,improve compliance and comfort and promote the improvement of patients' quality of life in patients with tumors receiving maintenance hemodialysis.
