摘要
为规范居民就医秩序,提高分级诊疗制度的实施效果,采用分层随机抽样方法对徐州市1 473名城乡居民就医机构选择进行分析。结果显示,城市居民患病就诊率高于农村,基层首诊率低于农村;影响城乡居民就医机构选择的共同因素包括是否知晓分级诊疗制度、年均月收入(P<0.05),城市居民还受医疗费用、文化程度的影响(P<0.05),农村居民还受职业、与基层医疗机构的距离、自评健康状况的影响(P<0.05)。因此,各级部门仍要增强政策宣传、合理配置资源、降低医疗费用、提升基层服务质量,以提高基层首诊率,提升制度实施效果。
To investigate urban and rural residents’choices of medical treatment and analyze the implementation effect of hierarchical diagnosis and treatment system,1,473residents were investigated by using stratified random sampling method in Xuzhou.The results showed that the medical treatment rate of urban residents was higher than rural residents.The rate of urban residents chosen community health service institutions was lower than rural residents.And the common influencing factors of urban and rural residents’health service institutions consisted of average monthly income and awareness of the system (P<0.05).Urban residents were also affected by medical expenses and educational levels(P< 0.05).Rural residents were also influenced by occupation,distance to primary medical institutions and self-perceived health status(P<0.05).Therefore,medical institutions at all levels should further enhance the propaganda of policy, allocate resources reasonably,reduce medical expenses,improve the quality of community health service institutions and the rate of first treatment in community health service institutions,and ensure the implementation effect of the hierarchical diagnosis and treatment system.
