摘要
目的应用鼻内镜下泪前隐窝入路的手术进路方法,处理上颌窦内的良性病变,避免了传统Caldwell-luc术式及鼻侧切开手术的带来的手术并发症,术后患者恢复快、痛苦小、治愈率高。方法鼻内镜下以泪前隐窝作为开放上颌窦手术的进路方法,术中仅需切开鼻腔外侧壁粘膜,磨除上颌窦内侧壁及梨状孔边缘部分骨质,便可以在鼻内镜辅助下彻底观察上颌窦的各个面,借助角度器械可彻底清理上颌窦内根蒂在各个壁来源的良性病变。结果对我科2010年11月~2016年06月收治的上颌窦含牙囊肿、原发于上颌窦乳头状瘤,顽固性的上颌窦息肉及部分上颌窦囊肿、霉菌、上颌窦异物等良性病变,行鼻内镜下泪前隐窝入路的手术,术中出血少,手术操作简单、术后并发症无,疗效明确。结论对于上颌窦良性病变,应用鼻内镜下泪前隐窝入路,既能彻底处理病变组织,又避免了传统手术带来的手术并发症,术中损伤小,术后恢复快,是治疗上颌窦良性病变的理想术式。
Objective To treat the benign lesions in the maxillary sinus by endoscopic surgery,and to avoid the complications of traditional Caldwell-luc and nasal incision.Symptoms, postoperative patients recover quickly,have less pain,and have a high cure rate.Methods The anterior lacrimal crypt was used as a method of open maxillary sinus surgery under nasal endoscopy.In the operation,only the nasal mucosa of the nasal cavity was cut,and the inner wall of the maxillary sinus and the bone of the edge of the pear-shaped hole were removed.Thoracic endoscopy-assisted observation of the various faces of the maxillary sinus, the use of angle instruments can completely clean the benign lesions of the maxillary sinus pedicle in each wall.Results The maxillary sinus treated in our department from November 2010 to June 2016 Intraoperative endoscopic surgery for the treatment of anterior maxillary sinus polyps and refractory maxillary sinus polyps and some maxillary sinus cysts,mold,maxillary sinus foreign body and other benign lesions.Less bleeding, simple operation,no postoperative complications,and clear curative effect.Conclusion For benign lesions of the maxillary sinus,the application of endoscopic anterior lacrimal cryptectomy can not only completely treat the diseased tissue,but also avoid the surgical complications caused by traditional surgery.The intraoperative injury is small and the postoperative recovery is fast.The ideal surgical procedure for the treatment of benign lesions of the maxillary sinus.
