摘要
目的:探讨从肝阳虚理论辨治恶性肿瘤术后并发抑郁的学术经验。方法:通过归纳与总结肝阳虚理论及恶性肿瘤术后并发抑郁的临床研究,结合古今名医相关论述,对肝阳虚理论辨治恶性肿瘤术后并发抑郁学术经验进行探讨,并举案例予以佐证。结果:恶性肿瘤患者术后并发抑郁随着癌症发病率上升而临床日益多见,而肝阳虚是恶性肿瘤术后并发抑郁的重要病因病机。恶性肿瘤患者发病前即存在正气亏虚之本,癌毒侵袭损伤正气,正气衰久致肝气受损,复因手术伐伤气血,心身遭创,阳气虚衰,肝阳益发亏虚致肝之疏泄功能失调,则气机失和,神气郁结,故恶性肿瘤术后患者易并发抑郁。基于“肝体阴而用阳”理论,首当补益肝阳,善补阳者必于阴中求阳,不忘滋肝阴以育肝阳;还应顺应肝性,养血以柔肝。结论:从肝阳虚理论辨治恶性肿瘤术后并发抑郁,取得了很好的临床疗效,值得深入探讨和推广。
Objective: To explore the academic experience of treating postoperative depression from the theory of liver-Yang deficiency. Methods: By summing up the theory of deficiency of liver-Yang and the clinical study of postoperative depression of malignant tumor,combined with the relevant exposition of ancient and modern famous doctors,the academic experience of treating postoperative depression of malignant tumor with liver-Yang deficiency theory was discussed. The case was also given to support the case. Results: Depression was more and more common in patients with malignant tumor after operation with the increasing incidence of cancer,and deficiency of liver-Yang was an important etiology and pathogenesis of depression after operation of malignant tumor. Before the onset of malignant tumor,there was the root of deficiency of Qi,the damage of Qi caused by cancer poison invasion,the damage of liver Qi caused by chronic failure of Qi,the injury of Qi and blood by surgery,the injury of heart and body,the deficiency of Yang Qi,the imbalance of the function of dredging the liver caused by deficiency of liver Yang. Therefore,patients with malignant tumor were prone to depression after operation. Based on the theory of"liver body Yin and using Yang",the first was to invigorate liver Yang,and those who were good to replenish Yang must seek Yang in Yin,did not forget to nourish liver Yin to nurture liver Yang,and should comply with liver nature and nourish blood to soften liver. Conclusion: The theory of liver-Yang deficiency is appllied in the treatment of malignant tumor complicated with depression has achieved good clinical effect,and it is worth further discussing and popularizing.
