摘要
目的:研究人附睾分泌蛋白(serum human epididymis protein,HE4)在卵巢低级别和高级别浆液性癌临床鉴别诊断中的价值。方法:按照两级分类法将经临床和病理确诊的95例卵巢浆液性癌分为低级别(Low-grade serous carcinoma,LGSC)和高级别(High-grade serous carcinoma,HGSC)。应用Elisa双抗体夹心法检测所有病例的HE4血清水平,比较两组间差异。免疫组化染色检测患者肿块石蜡切片中HE4和P53的表达水平,分析比较两组的阳性表达率有无差异。结果:95例卵巢浆液性癌分为LGSC 25例,HGSC 70例,处于FIGO(Ⅰ~Ⅱ)期LGSC 18例(18/25),HGSC 10例(10/70)。HE4和P53在LGSC和HGSC患者肿瘤石蜡切片中的阳性表达率分别为32.00%vs 95.71%和12.00%vs 85.71%(P<0.05)。LGSC患者的HE4血清水平(121.97±18.17)pmol/L明显低于HGSC(305.37±40.41)pmol/L,二者比较差异具有显著性意义(P<0.01)。但是对于FIGO分期均为(Ⅰ~Ⅱ)期的LGSC和HGSC患者,其HE4血清水平差异无统计学意义(85.53±5.64)vs(90.50±6.14),P>0.05)。结论:检测HE4血清水平和组织中的蛋白表达水平有助于LGSC和HGSC的临床鉴别诊断,但血清HE4水平对于二者同为(Ⅰ~Ⅱ)期时的鉴别诊断作用还有待进一步研究。
Objective:This study is aimed to investigate the value of serum human epididymis protein 4(HE4)in clinical differential diagnosis of low-grade serous(LGSC)and high-grade serous carcinoma(HGSC).Method:A total of 95 patients who were diagnosed with ovarian serous carcinoma were divided into LGSC and HGSC by the two-tier grade system.Serum levels of HE4 were measured by double antibody sandwich method to compare the difference in two groups.HE4 and TP53 protein in cancer tissue were measured by immunohistochemical method,the positive rate of which was compared in two groups.Results:There are 25 LGSC and 70 HGSC in all 95 ovarian serous carcinoma,among which there were 18(18/25)patients of LGSC in FIGO stage(Ⅰ~Ⅱ)and 10(10/70)patients of HGSC in the same stage.The positive rate of HE4 and TP53 protein between LGSC and HGSC tissue were 32.00% vs 95.71% and12.00%vs 85.71%respectively(P<0.05).There was significant difference in serum HE4 levels between LGSC and HGSC(121.97±18.17)pmol/L vs(305.37±40.41)pmol/L,P<0.01,but the result was different within FIGO stage(Ⅰ~Ⅱ).Conclusions:The detection of HE4 serum level and protein expression level in tissues are helpful for the clinical differential diagnosis of LGSC and HGSC.However,the serum HE4 level is still to be further studied for the differential diagnosis when both of them are stage(Ⅰ~Ⅱ).
