摘要
背景疼痛的评估往往依赖患者的主观描述。手术中常规的疼痛评估手段往往依靠一些生命体征或临床经验,这些手段往往缺乏特异性和敏感性。目的对近年来提出的疼痛监测方法手术体积描记指数(surgical pleth index,SPI)的研究进行分析并综述。内容阐述SPI监测疼痛的原理和优势,从指导镇痛药物使用,评估伤害性应激抗伤害性应激平衡(nociception-anti-nociception balance,BANA),预测术后疼痛水平及影响因素和适用范围等方面总结了SPI监测在围手术期中的应用。趋向SPI监测为临床工作中围手术期疼痛水平的评估提供了新思路和方向,有广阔的应用前景。
Background Pain in awake patients can be assessed on the basis of their subjective description. But the pain assessment during surgery often depends on several vital signs and practitioners' clinical experience, and often lacks specificity and sensitivity. Objective To analyze and summarize recent studies in operative pain monitoring with surgical pleth index (SPI). Content This article presents the procedures and effectiveness of SPI in monitoring pain, and summarizes the application of SPI in perioperative pain monitoring. SPI can be used to assess the balance between nociception and anti-nociception to guide the use of analgesic drugs, to predict postoperative pain levels, and to analyze influencing factors of pain symptoms, as well. Trend SPI monitoring provides a new avenue for the assessment of perioperative pain levels.
