摘要
目的总结早期先天性梅毒(CS)新生儿的临床症状及先天性骨梅毒的影像学表现,提高对该病的认识。方法收集2015年至今经临床及血清学证实的早期CS新生儿患者23例,均摄胸腹部平片及四肢长骨片。结果23例早期CS患儿中,4例出现先天性骨梅毒,骨膜炎者4例,干骺端炎3例,2例脊柱椎体、1例跗骨及3例骨盆出现类似干骺端炎的征象,2例锁骨骨膜炎。上述4例患儿母亲未经过正规治疗,均为早产儿、低体重,且血清滴度较高,并多伴其他临床症状,其中1例患儿预后较差。结论早期CS出现先天性骨梅毒的患儿较多合并其他临床症状,且血清滴度较高。熟悉早期CS的临床表现及骨骼病变、正确的产前梅毒筛查、规范的治疗,对确诊血清阳性妇女、防止垂直传播及改善患儿预后非常重要。
Objective To summarize the clinical symptoms of early congenital syphilis (CS)in newborns and the imaging findings of CS with bone to raise the awareness of the disease.Methods 23 newborns confirmed with early CS from 2015 to the present,clinically and serologically,were enrolled,and X-ray radiographs of the chestabdomen and long bones were taken.Results Among the 23 newborns with early CS,CS with bone radiographic examination abnormalities were found in 4 cases.4 cases suffered periostitis,3 cases with osteochondritis and periostitis at metaphyseal level.Vertebraes in 2 cases,tarsus in 1 case,and pelvis in 3 cases were similar to metaphysis.Clavicular periostitis were found in 2 cases.The 4 cases,whose mothers not treated adequately,were premature infants,low body weight,higher serum dilutions and with more other clinical symptoms,including 1patient with poor prognosis.Conclusion Early CS patients with bone abnormalities have many more clinical symptoms and higher serum titers.Familiar with the clinical manifestations and X-ray radiographs of long-bone lesions of early CS,correcting antenatal syphilis screening are important to identify serum-positive women,to give them adequate treatment to prevent the risk of vertical transmission and improve the prognosis of newborns.
