摘要
目的观察米索前列醇联合阴超在无痛人工流产中的应用效果,以降低人工流产的手术风险。方法随机抽取2016年6月—2017年6月在南京市六合区妇幼保健所进行无痛人工流产手术者200例,分为干预组和对照组各100例。干预组术前2 h舌下含服米索前列醇0. 4 mg,术中阴道超声引导。对照组未用药物及超声进行干预,直接进行无痛人工流产术。统计两组手术效果及术中、术后并发症等情况。结果干预组的宫颈软化程度好,术中出血量少,子宫收缩佳,术中子宫穿孔,漏吸、组织残留及术后月经改变等围手术期并发症发生率低,与对照组相比差异有统计学意义(P <0. 05)。结论无痛人工流产术前舌下含服米索前列醇可有效地软化宫颈,有利于宫颈扩张及手术器械进出宫腔,同时促进子宫收缩,减少术中出血。术中阴道超声引导人工流产,提高手术的精准性,减少了盲目性及手术损伤,两种方法联合用于无痛人工流产,降低了手术的风险,值得在临床推广使用。
Objective To observe the effect of misoprostol combined with transvaginal ultrasound in painless induced abortion,in order to reduce the risk of abortion.Methods Totally 200cases of painless abortion operated by June 2016 to June 2017were randomly selected and divided into 100cases in the intervention group and the control group.In the intervention group,2hours before the operation,misoprostol 0.4 mg was taken under the tongue,and the vaginal ultrasound guidance was performed during the operation.The control group did not use drugs and ultrasound intervention to direct the painless induced abortion.The results of the two groups of surgery and the intraoperative and postoperative complications were analyzed.Results In the intervention group,the degree of cervical softening,bleeding volume and uterine contraction were good,and the incidence of perioperative complications such as uterine perforation,aspiration, tissue residue and postoperative menstrual change was low.There was a significant difference between the intervention group and the control group (P <0.05).Conclusion Sublingual misoprostol can effectively soften the cervix before painless artificial abortion,which is conducive to cervical dilatation and surgical instruments to enter the uterine cavity, while promoting uterine contraction and reducing intraoperative bleeding.Intraoperative vaginal ultrasound guided abortion,improve the accuracy of surgery,reduce blindness and surgical injury,the two methods used for painless induced abortion,reduce the risk of surgery,it is worth promoting in clinical practice.
