摘要
目的观察N-乙酰半胱氨酸(N-Acetyl-cysteine,NAC)对肥胖哮喘患者气道炎症及病情控制水平的影响。方法选取2014年6月至2016年6月大连大学附属新华医院呼吸科就诊的肥胖哮喘患者60例,随机分为治疗组和对照组,每组30例。对照组给予哮喘常规治疗(表面激素/长效β2受体激动剂),治疗组在常规治疗基础上加服NAC泡腾片600mg/d,疗程12周。观察两组患者治疗前后血清肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、白细胞介素6(interleukin-6,IL-6)及哮喘控制测试(asthma control test,ACT)评分、呼气峰值流量(peakexpiratoryflow,PEF)等指标的变化情况。结果治疗组患者治疗后血清TNF-α、IL-6较治疗前明显降低(P<0.05),ACT评分、PEF较治疗前明显提高(P<0.05)。对照组患者治疗前后上述指标比较差异均无显著性(P>0.05)。两组治疗后上述指标比较差异有显著性(P<0.05)。结论 NAC可通过抑制氧化应激反应,减轻哮喘气道炎症,提高哮喘控制水平,改善肥胖哮喘患者的治疗效果。
Objective To investigate the effect of N-acetylcysteine on the airway inflammation and the disease control of obeseasthmatic patients. Method From June 2014 to June 2016, 60 obese asthmatic patients were selected in the department of respiratory,Affiliated Xinhua Hospital Of Dalian University. They were randomly divided into treatment group(30 cases) and control group(30cases). The patients in the control group were treated with conventional therapy(ICS/LABA). The patients in the treatment group addedN-acetylcysteine(600mg/d) on the basis of the control group. Patients in two groups were treated for 12 weeks. The serum contents ofTNF-α, IL-6, and ACT score, PEF in two groups before and after treatment were observed. Result After treatment, The serum contents ofTNF-α, IL-6 were significantly decreased, and ACT score, PEF were significantly increased in the treatment group (P<0.05). There wereno significant difference before and after treatment in the control group(P>0.05), and there were significant differences after treatmentbetween two groups(P<0.05). Conclusion N-acetylcysteine can relieve the airway inflammation, raise the level of asthma control, andimprove therapeutic effect of obese asthmatic patients by inhibiting oxidative stress reaction.
